摘要
目的探讨额颞叶胶质瘤病人量化脑电参数的特点。方法回顾性分析33例弥漫性脑胶质瘤病人的临床及术前脑电图资料,按肿瘤是否累及额、颞叶分为额叶病例组与对照组,颞叶病例组与对照组。筛选无明显伪差的特异性时间段,提取α/δ波比率(alpha-delta ratio,ADR)、相对频带能量及光谱边界等量化脑电参数,取病侧与健侧对应导联参数的比值进行统计分析。结果额叶病例组量化脑电特征的病/健侧比值与额叶对照组的左/右侧比值差异均无统计学意义(P>0.05))。与颞叶对照组比较,颞叶病例组F7~F8导联θ频段相对能量的病/健侧比值偏低(P=0.041);T3~T4导联δ频段相对能量的病/健侧比值偏高(P=0.003),α频段相对能量的病/健侧比值偏低(P=0.046),ADR值的病/健侧比值偏低(P=0.006);T5~T6导联β频段相对能量的病/健侧比值偏低(P=0.021),光谱边界的病/健侧比值偏低(P=0.005)。结论定量脑电特征可用于胶质瘤及其相关癫癎的定位、定侧诊断。
Objective To investigate the characteristics of quantitative electroencephalogram(EEG)parameters in patients with frontotemporal gliomas.Methods Clinical and preoperative EEG data of 33 patients with diffuse gliomas were analyzed retrospectively,and the patients were divided into temporal case group,temporal control group,frontal case group and frontal control group according to whether the tumor involved the frontal or temporal lobes.EEG data were screened for specific time periods without obvious artifacts,and quantitative EEG parameters including alpha-delta ratio(ADR),relative frequency band energy and spectral boundary were extracted.Statistical analysis was performed using the ratio of the corresponding parameters of the affected side to the healthy side.Results No significant differences were identified in quantitative EEG characteristics between the affected/healthy side ratio of frontal case group and the left/right ratio of frontal control group(P>0.05).Compared with the temporal control group,the affected/healthy side ratio ofθband relative energy was relatively low in lead F7 to F8(P=0.041),the affected/healthy side ratio ofδband relative energy was relatively high(P=0.003),and relatively low forαband relative energy(P=0.046)and ADR value(P=0.006)in lead T3 to T4,and the affected/healthy side ratios ofβband relative energy(P=0.021)and spectral boundary(P=0.005)were relatively low in lead T5 to T6.Conclusion Quantitative EEG characteristics can be used for the localization and lateral diagnoses of glioma and its related epilepsy.
