摘要
目的探讨调Q开关1064 nm Nd:YAG激光联合维A酸乳膏治疗黄褐斑的疗效及其抗氧化作用。方法自2015年10月至2018年9月,对收治的108例黄褐斑患者按随机数字表法分为对照组和观察组,每组54例。对照组采用调Q开关1064 nm Nd:YAG激光治疗仪治疗,观察组在对照组治疗基础上于激光治疗后2周给予0.025%维A酸乳膏外涂治疗。比较两组患者的总有效率、皮损面积及皮损颜色评分、不良反应以及血清超氧化物歧化酶(superoxide dismutase, SOD)、丙二醛(malondialdehyde, MDA)、过氧化氢酶(catalase, CAT)水平。结果观察组的总有效率(87.03%)高于对照组(66.67%);两组治疗后的皮损面积和皮损颜色评分较均治疗前显著下降(P<0.01),且观察组治疗后的评分明显低于对照组(P<0.01);对照组和观察组色素沉着发生率分别为14.81%和1.85%(P<0.05);观察组治疗后血清SOD、CAT水平较治疗前明显升高,MDA水平显著降低(P<0.01);且观察组治疗后血清SOD、MDA、CAT的水平改善均优于对照组(P<0.01)。结论观察组治疗对黄褐斑的疗效优于对照组,且对抗氧化作用调节更好,值得临床推广应用。
Objective To explore the efficacy and antioxidant effect of Q switched 1064 nm Nd : YAG laser combined with vitamin A acid cream in the treatment of chloasma. Methods From October 2015 to September 2018, 108 patients with chloasma were divided into the control group and observation group according to the random number table, 54 patients in each group. The Q switched 1064 nm Nd: YAG laser therapy was performed in the control group and 0.025% vitamin A acid cream was applied at 2 weeks after laser treatment in the observation group. The total effective rate, scores of skin lesion area and color, adverse reactions and serum levels of superoxide dismutase(SOD), malondialdehyde(MDA) and catalase(CAT) between two groups were compared. Results The total effective rate of observation group(87.03%) was higher than that of the control group(66.67%). The scores of skin lesion area and color in both groups were significantly decreased after treatment(P<0.01), and the scores in the observation group were significantly lower than those in the control group(P<0.01). The incidence rate of pigmentation was 14.81% in the control group and 1.85% in the observation group(P<0.05). The serum level of SOD and CAT in the observation group after treatment was significantly higher than that before the treatment, and the serum level of MDA was significantly decreased(P<0.01). The improvement of serum level of SOD, MDA and CAT in the observation group after treatment was better than that in the control group(P<0.01). Conclusion The efficacy of chloasma and antioxidative effect in the observation group is better than that in the control group. It should be recommended to popularize in clinic.
