摘要
目的分析活血通络方联合西药治疗冠状动脉粥样硬化性心脏血瘀证的临床疗效。方法将医院2018年1月-2019年6月纳入的冠心病(血瘀证)患者94例按随机数字表法分为观察组和对照组各47例,对照组单硝酸异山梨酯,40 mg/次,1次/d;阿托伐他汀钙,30 mg/次,1次/d,连续用药4周。观察组在对照组基础上加用活血通络方(桃仁8 g,丹参10 g,川芎12 g,赤芍10 g,红花10 g,当归12 g,水蛭0.5 g,地龙15 g),水煎400 mL,1剂/d,早晚温服,连续用药4周。观测两组治疗后中医证候积分、临床疗效、同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)。结果治疗后,观察组临床总有效率显著高于对照组(P<0.05)。治疗前,两组Hcy、hs-CRP比较,差异无统计学意义(P>0.05),具有可比性;治疗后,两组Hcy、hs-CRP均降低(P<0.05),观察组低于对照组(P<0.05)治疗前,两组中医证候积分比较,差异无统计学意义(P>0.05),具有可比性;治疗后,两组中医证候积分均降低(P<0.05),观察组低于对照组(P<0.05)。结论活血通络方联合西药治疗冠状动脉粥样硬化性心脏血瘀证可提高治疗效果,改善临床症状,改善生化指标,值得推广。
Objective To analyze the clinical effect of Huoxue Tongluo decoction combined with western medicine on coronary atherosclerotic heart blood stasis syndrome.Methods 94 patients with coronary heart disease(blood stasis syndrome)included in our hospital from January 2018 to June 2019 were randomly divided into observation group and control group with 47 cases each.Isosorbide mononitrate in the control group was 40 mg/time,once/d;atorvastatin calcium was 30 mg/time,once/d for 4 weeks.In the observation group,on the basis of the control group,Huoxue Tongluo formula(Taoren 8 g,Danshen 10 g,Chuanxiong 12 g,red peony 10 g,safflower 10 g,angelica 12 g,leech 0.5 g,earthworm 15 g)was added,decoction 400 mL,1 dose/d,warm in the morning and evening,for 4 weeks.After treatment,TCM syndrome score,clinical effect,homocysteine(Hcy),high-sensitivity C-reactive protein(hs CRP)were observed.Results After treatment,the total clinical effective rate of the observation group was significantly higher than that of the control group(P<0.05).Before the treatment,Hcy and hs CRP of the two groups had no significant difference(P>0.05),which was comparable;after the treatment,Hcy and hs CRP of the two groups decreased(P<0.05),and the observation group was lower than the control group(P<0.05).Before the treatment,there was no significant difference between the two groups(P>0.05),which was comparable;after the treatment,the scores of the two groups were reduced(P<0.05),and the observation group was lower than the control group(P<0.05).Conclusion Huoxue Tongluo decoction combined with western medicine can improve the therapeutic effect,clinical symptoms and biochemical indexes of coronary atherosclerotic heart blood stasis syndrome,which is worth popularizing.
