摘要
目的探讨坏死性筋膜炎(necrotizing fasciitis,NF)的治疗体会。方法对4例NF患者在早期积极清创、引流、使用有效的抗生素,纠正电解质紊乱,治疗原发病及加强生命支持等综合治疗下,尽早行高压氧治疗。结果4例患者的重症感染能有效控制,创面经过拉拢缝合或皮瓣修复后均治愈出院。结论对于NF患者,经多学科综合治疗生命征平稳后尽早行高压力高压氧治疗,可减轻全身中毒反应,有效控制感染,提高抢救成功率,降低并发症的发生率。
Objective To explore the experience in the treatment of necrotizing fasciitis(NF).Methods Four patients with NF were treated with hyperbaric oxygen as soon as possible after the comprehensive treatment including early active debridement,drainage,using effective antibiotics,correcting electrolyte disturbance,treating primary disease and strengthening life support.Results The severe infection in 4 patients could be effectively controlled,and all of them were cured and discharged after wound suture or flap repair.Conclusion High pressure hyperbaric oxygen therapy should be given as soon as possible for NF patients with stable vital signs after multidisciplinary comprehensive treatment,which can reduce systemic toxic reaction,effectively control infection,improve the success rate of rescue,as well as reduce the incidence of complications.
