摘要
目的观察布托啡诺联合超声引导腹横肌平面阻滞(TAPB)对腹腔镜子宫肌瘤切除术患者苏醒期的影响。方法选择60例ASA分级Ⅰ~Ⅱ级择期行腹腔镜子宫肌瘤切除术的患者,随机分为布托啡诺组(B1组)、双侧腹横肌平面阻滞组(B2组)和布托啡诺+双侧腹横肌平面阻滞组(B3组),B1组在手术结束前30min静脉注射布托啡诺1mg,B2组在麻醉诱导后超声引导下行双侧腹横肌平面阻滞,B3组在麻醉诱导后超声引导下行双侧腹横肌平面阻滞,手术结束前30min静脉注射布托啡诺1mg,观察3组在苏醒期的循环情况、苏醒时间及拔管时间,同时观察术后疼痛视觉模拟评分(VAS)和舒适度评分(BCS)。结果B3组围拔管期各时点的血压、心率及术后VAS评分等均显著低于其他两组,而BCS评分显著高于其他两组(P<0.05),B3组苏醒时间及拔管时间与其他两组比较差异无统计学意义(P>0.05)。结论腹腔镜子宫肌瘤切除术患者使用布托啡诺联合超声引导腹横肌平面阻滞可使全麻苏醒期患者的循环更加稳定,术后疼痛明显减轻,且不会引起苏醒延迟。
Objective To observe the effect of butorphanol combined with ultrasound guided transversus abdominis plane block on awakening period of patients undergoing laparoscopic myomectomy.Methods Sixty ASAⅠ~Ⅱpatients who were scheduled for laparoscopic myomectomy were randomly divided into three groups including butorphanol group(group B1),TAPB group(group B2),butorphanol combined with TAPB group(group B3).Group B1 was intravenously injected with lmg butorphanol 30 minutes before the end of operation,group B2 was performed with the ultrasound guided TAPB after anesthesia induction,group B3 was intravenously injected with lmg butorphanol30 minutes before the end of operation and combined with the ultrasound guided TAPB after anesthesia induction.Recorded the SBP,DBP,HR,awakening time and extubation time,pain visual scale(VAS)scores and body comfort scale(BCS)scores during the recovery period.Results The blood pressure,heart rate,VAS scores of group B3 were significantly lower than that of group B1 and group B2,while body comfort scale(BCS)scores was significantly higher(P<0.05).There was no significant difference on the awakening time and extubation time among the three groups(P>0.05).Conclusion Butorphanol combined with ultrasound guided transversus abdominis plane block can better maintain the hemodynamics stability and decrease postoperative pain,and not affect the recovery time of anesthesia in patients with laparoscopic myomectomy.
