摘要
目的探讨微种植体支抗与常规矫正在口腔正畸患者中的临床治疗效果及其时间-疗效变化。方法选取2014年12月~2017年12月在本院口腔科诊治的成人安氏Ⅱ类1分类错[牙合]畸形的患者60例,根据患者支抗使用情况分为微种植体支抗组(n=30)和常规矫正组(n=30)。两组患者均使用MBT直丝弓矫治技术。治疗前后拍摄头颅侧位片并测量,比较颌骨、牙齿、软组织各指标的改变,得到其时间-疗效变化。结果两组患者治疗后SNA角、SNB角、ANB角都不同程度减小,GoGn-SN角、OP-SN角轻微增大。治疗后两组患者牙Ul-Apg距、U6-Ptm距及Z角(Z angle)的组间差异有统计学意义(P <0.05)。与常规矫正组比较,微种植体支抗组疗程缩短,差异有统计学意义(P <0.05)。两组患者OP-SN、U1-Ptm和Z angle在矫正24个月后比较差异有统计学意义,OP-SN和U1-Apg第12个月和24个月两组比较差异有统计学意义(P <0.05)。结论微种植体支抗治疗安氏Ⅱ类1分类错[牙合]畸形能获得显著的上颌切牙内收、上下唇突度减小的效果,可以缩短疗程。
Objective To investigate the clinical therapeutic efficacy and time-efficacy changes of micro-implant anchorage and routine correction in orthodontic patients.Methods A total of 60 adult patients with Angle's classⅡ,subclass 1 malocclusion diagnosed and admitted to the stomatology department of our hospital from December 2014 to December 2017 were selected and divided into the micro-implant anchorage group(n=30)and the routine correction group(n=30)according to the use of anchorage.Both groups were treated with MBT straight arch wire techniques.Lateral cephalic images were taken and measured before and after treatment to compare the changes of jaw,teeth and soft tissue indexes and to obtain the time-efficacy changes.Results After treatment,SNA angle,SNB angle and ANB angle decreased with different degrees,while GoGn-SN angle and OP-SN angle slightly increased in both groups.After treatment,there were statistically significant differences between the two groups in Ul-Apg distance,U6-Ptm distance and Z angle(P<0.05).Compared with the routine correction group,the treatment course in the micro-implant anchorage group was shorter,and the differences were statistically significant(P<0.05).There were significant differences in OP-SN,U1-Ptm and Z angle between the two groups after 24 months of correction,and the differences between OP-SN and U1-Apg in the 12th and 24th months were statistically significant(P<0.05).Conclusion Micro-implant anchorage for Angle's classⅡ,subclass 1 malocclusion achieves remarkable efficacies of maxillary incisor adduction and reduction of protrusion of upper and lower lip and shortens the treatment course.
