单微导管分区成蓝技术栓塞颅内分叶状破裂动脉瘤体会 被引量:1

Clinical Application Analysis of a Single Microcatheter Divided into Blue Technology on Embolization of Intracranial Lobulated Ruptured Aneurysm

摘要 目的:探讨单根微导管分区成蓝技术栓塞颅内分叶状破裂动脉瘤的临床效果、安全性及注意事项。方法:应用单微导管分区成蓝技术对83例分叶状颅内破裂动脉瘤进行了血管内介入治疗,对其栓塞策略(根据载瘤动脉的走形、动脉瘤分叶、大小及朝向对微导管头端进行精准塑形)、材料选择、手术要点及栓塞效果进行进行回顾分析。结果:83例分叶状动脉瘤中宽颈67例,支架辅助栓塞26例(38.81%,26/67),83例分叶状颅内破裂动脉瘤栓塞治疗均获成功,其中2例宽颈多分叶动脉瘤支架辅助栓塞时微导管脱出,给于补救性微导管穿支架网眼再次进入瘤囊技术完成栓塞。单根微导管分区成蓝技术栓塞颅内分叶状破裂动脉瘤的技术成功率为97.6%,完全栓塞75例(90.36%),次全栓塞8例(9.64%),术中1例发生动脉瘤破裂,1例术后发生血栓栓塞症状。结论:只要能根据载瘤动脉的走形、动脉瘤分叶的大小及朝向对微导管头端进行精准塑形,单根微导管分区成蓝技术能够安全、有效地栓塞颅内分叶状破裂动脉瘤,且能够明显降低急性期支架使用率。
