摘要
目的探讨NF-κB-iNOS mRNA-NO信号通路在糖尿病痛性神经病变(PDN)中的作用。方法选取2017年2~12月于火箭军特色医学中心内分泌科住院治疗的114例T2DM患者,分为单纯T2DM组(T2DM,n=33)、PDN组(n=30)及糖尿病神经病变无疼痛组(NPDN,n=51)。同期选取本院健康体检人群30名为正常对照(NC)组。比较各组一般资料及HbA1c等生化指标。采用Elisa试剂盒检测血清核因子κB(NF-κB)水平及一氧化氮(NO)含量;实时荧光定量PCR技术检测外周血单个核细胞中诱导型一氧化氮合酶(iNOS)基因表达水平。结果PDN组NF-κB、iNOS mRNA水平较NPDN、T2DM、NC组升高(P<0.05);T2DM、NPDN组NO含量较NC组降低(P<0.05);PDN组NO含量高于其余3组(P<0.05)。Pearson相关分析提示,NF-κB水平与DM病程、TG、HbA1c呈正相关,iNOS mRNA水平与TG、HbA1c呈正相关(P<0.05或P<0.01)。Logistic回归分析显示,NF-κB、iNOS mRNA、NO是PDN的影响因素。结论NF-κB-iNOS mRNA-NO信号通路可能在PDN中发挥重要作用。
Objective To investigate the role of NF-κB-iNOS mRNA-NO signaling pathway in diabetic painful neuropathy(PDN).Methods 114 T2DM patients who were hospitalized in the Endocri-nology department of Rocket Army Characteristic Medical Center from February to December 2017 were selected and divided into simple T2DM group(T2DM,n=33),PDN group(n=30)and diabetic neuropathy without pain group(NPDN,n=51).30 healthy people were selected as normal control(NC)group.The general data and biochemical indexes such as HbA1c in each group were compared.ELISA kit was used to detect the level of serum nuclear factor kappa B(NF-κB)and nitric oxide(NO),and real-time fluorescence quantitative PCR was used to detect the expression of inducible nitric oxide synthase(iNOS)gene in peripheral blood mononuclear cells.Results The levels of NF-κB and iNOS mRNA in PDN group were higher than those in NPDN,T2DM and NC groups(P<0.05).NO content in T2DM and NPDN group was lower than that in NC group(P<0.05).NO content in PDN group was higher than that in other three groups(P<0.05).Pearson correlation analysis showed that the level of NF-κB was positively correlated with DM duration,TG and HbA1c(P<0.05 or P<0.01),and the level of iNOS mRNA was positively correlated with TG and HbA1c(P<0.05 or P<0.01).Logistic regression analysis showed that NF-κB,iNOS mRNA and NO were the influencing factors of PDN.Conclusion The NF-κB-iNOS mRNA-NO pathway may play an important role in the development of PDN.
