摘要
目的探讨甲状腺功能减退对妊娠糖尿病发病率及妊娠结局的影响评价。方法采用掷骰子方法设计,将2018年1月—2019年6月期间在该院产科的健康孕妇50名设为对照组,同期将在该院产科的妊娠糖尿病合并甲状腺功能减退者孕妇50例设为实验组;两组孕妇均接受甲状腺功能检查方法,均接受口服葡萄糖耐量试验(OGTT)方法。结果实验组的0 hOGTT血糖、1 hOGTT血糖、2 hOGTT血糖较对照组升高(P<0.05);实验组的FBG、2 hPG、HbAlc较对照组升高(P<0.05);实验组的TSH、TPOAb较对照组升高(P<0.05);实验组的剖宫产发生率较对照组患者升高(P<0.05);实验组的先兆流产发生率、早产发生率较对照组升高(P<0.05)。结论妊娠糖尿病合并甲状腺功能减退者孕妇可影响其妊娠结局,因此临床需加强重视,积极改善其甲状腺功能指标,从而有效降低相关不良妊娠结局的发生。
Objective To evaluate the impact of hypothyroidism on the incidence of gestational diabetes and pregnancy outcome.Methods Using the dice method design,50 healthy pregnant women who were in the obstetrics department of the hospital from January 2018 to June 2019 were set as the control group,and the gestational diabetes in the obstetrics department of the hospital from the same period,50 cases of pregnant women with hypothyroidism were set as the experimental group;both groups of pregnant women received thyroid function test methods,and both received oral glucose tolerance test(OGTT)method.Results The blood glucose of 0 hOGTT,1 hOGTT,and 2 hOGTT of the experimental group were higher than those of the control group(P<0.05);the FPG,2 hPG,HbAlc of the experimental group were higher than those of the control group(P<0.05);the TSH and TPOAb of the experimental group were higher than those of the control Group increased(P<0.05);the incidence of cesarean section in the experimental group was higher than that in the control group(P<0.05);the incidence of threatened abortion and preterm delivery in the experimental group was higher than that of the control group(P<0.05).Conclusion Pregnant women with gestational diabetes and hypothyroidism can affect their pregnancy outcome.Therefore,it is necessary to pay more attention to the clinic and actively improve their thyroid function indicators,so as to effectively reduce the occurrence of related adverse pregnancy outcomes.
