尿毒症采取不同血流量血液灌流联合血液透析疗效分析 被引量:1

Effect of Different Blood Flow Hemoperfusion Combined with Hemodialysis on Uremia

摘要 目的观察对比不同血流量血液灌流联合血液透析治疗尿毒症的疗效。方法选取2016年4月至2019年5月在甘肃省陇南市第一人民医院行血液灌流联合血液透析治疗的96例尿毒症患者作为研究对象,依据奇偶数分组法将其分为观察组和对照组,每组48例。对照组采取血流量为200 mL/min进行治疗,观察组血流量设定为250 mL/min,比较两组治疗前后的生活指标、甲状腺旁激素(PTH)清除率、尿素清除指数(Kt/V)、疗效、透析器凝血发生率。结果两组治疗后的生化指标如甲状腺旁激素(iPTH)、血尿素氮(BUN)、血肌酐(SCr)、β2-微球蛋白(β2-MG)较本组治疗前比较均有改善,差异有统计学意义(P<0.05),但组间比较差异无统计学意义(P>0.05);两组患者PTH清除率和Kt/V值比较,差异无统计学意义(P>0.05);两组治疗有效率相比,差异无统计学意义(P>0.05);观察组透析器凝血发生率为0.00%低于对照组的12.50%,差异有统计学意义(P<0.05)。结论对进行血液灌流联合血液透析治疗尿毒症的患者,选择250 mL/min血流量血液灌流,不会对疗效产生影响,但能降低透析器凝血发生率。 Objective To observe and compare the efficacy of different blood flow hemoperfusion combined with hemodialysis in the treatment of uremia.Methods A total of 96 patients with uremia who received hemoperfusion combined with hemodialysis in the First People's Hospital,Longnan City,from April 2016 to May 2019 were selected as study subjects,and were divided into observation group and control group according to the odd even number grouping method,with 48 patients in each group.The blood flow in the control group was set at 200 mL/min for treatment,and the blood flow in the observation group was set at 250 mL/min.Comparison was made between the two groups before and after treatment in terms of life indicators,parathyroid hormone(PTH)clearance rate,urea clearance index(Kt/V),curative effect,and the incidence of dialyzer coagulation.Results The biochemical indexes such as iPTH,BUN,SCr,β2-MG were improved after treatment,and the difference was statistically significant(P<0.05).After treatment,there was no significant difference between the two groups(P>0.05).There was no statistically significant difference between the two groups(P>0.05).The incidence of dialyzer coagulation in the observation group was 0.00%lower than that in the control group 12.50%,with statistically significant difference(P<0.05).Conclusion For patients with uremia treated by hemoperfusion combined with hemodialysis,250 mL/min blood flow hemoperfusion will not affect the curative effect,but can reduce the incidence of dialyzer coagulation.
