地佐辛复合丙泊酚对无痛人流孕妇呼吸循环及苏醒时间的影响 被引量:8

摘要 目的:观察地佐辛复合丙泊酚对无痛人流孕妇呼吸循环及苏醒时间的影响。方法:选择2019年6月-2020年6月于本院行无痛人流手术的76例早孕患者,按随机数字表法分为对照组和观察组各38例。术前6 h禁食、禁饮,患者取截石位,手术过程中监测心率、呼吸、血氧饱和度等生命体征,建立静脉通道,予以3~5 L/min吸氧。对照组先予以芬太尼静脉注射,剂量为1.5μg/kg;观察组先予以地佐辛静脉推注,剂量为0.1 mg/kg。待手术部位消毒铺单完毕后,两组均再给予丙泊酚静脉推注,剂量为1.5 mg/kg,患者睫毛反射消失即开始手术,术中若出现身体扭动、皱眉等手术不耐受反应,可追加丙泊酚0.5~1.0 mg/kg。比较两组呼吸循环监测情况、苏醒时间、定向力恢复时间、疼痛(VAS)评分及不良反应。结果:麻醉前,两组平均动脉压(MAP)、心率(HR)、血氧饱和度(SpO2)比较,差异无统计学意义(P>0.05);麻醉2 min时,两组MAP、HR、SpO2较麻醉前下降(P<0.05),且观察组MAP、HR、SpO2水平高于对照组(P<0.05);苏醒时,两组MAP、HR比较,差异无统计学意义(P>0.05),但观察组SpO2高于对照组,差异有统计学意义(P<0.05)。观察组术后苏醒时间、恢复定向力时间短于对照组,苏醒时VAS评分低于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率略低于对照组,差异无统计学意义(P>0.05)。结论:地佐辛复合丙泊酚能够保持无痛人流孕妇呼吸循环平稳,缩短术后苏醒时间,且术后疼痛轻、不良反应少。 Objective:To observe the effect of dezocine combined with propofol on respiratory circulation and recovery time of pregnant women with painless induced abortion. Methods: 76 cases of early pregnancy patients undergoing painless abortion in our hospital from June 2019 to June 2020 were randomly divided into two groups,38 cases in each group. Fasting and drinking were forbidden 6 hours before the operation. The patients took the lithotomy position. During the operation,vital signs such as heart rate,respiration and blood oxygen saturation were monitored. Venous channels were established and oxygen was inhaled at 3 ~ 5 L/min. The control group was anesthetized with fentanyl combined with propofol,and the observation group was anesthetized with dezocine combined with propofol. The control group was anesthetized with fentanyl(1.5 μg/kg),while the observation group was anesthetized with dezocine(0.1 mg/kg). After the disinfection of the surgical site,propofol was give intravenously at the dose of 1.5 mg/kg. The patients started the operation when the eyelash reflex disappeared. If there were some intolerable reactions such as body twisting and frowning,propofol (0.5 ~ 1.0 mg/kg) could be added. Respiratory and circulatory monitoring,recovery time of orientation,VAS score and adverse reactions were compared between the two groups. Results: Before anesthesia,there was no significant difference in MAP,HR and SpO2 between the two groups(P > 0.05). At 2 min of anesthesia,MAP,HR and SpO2 in both groups were decreased compared with those before anesthesia,and the levels of them in the observation group were higher than those in the control group. At the time of recovery,there was no significant difference in MAP and HR(P > 0.05),but the SpO2 in the observation group was higher than that in the control group(P < 0.05). The recovery time and recovery time of orientation in the observation group were shorter than those in the control group,and the VAS score was lower in the observation group than in the control group(P < 0.05). The incidence of adverse reactions in the observation group was slightly lower than that in the control group(P > 0.05). Conclusion: Dezocine combined with propofol can keep the respiratory circulation of pregnant women with painless induced abortion stable,shorten the postoperative recovery time,and reduce the postoperative pain and adverse reactions.
