ICU留置导尿管质量控制管理在尿路感染风险预测模型中的应用 被引量:1

摘要 目的构建尿路感染风险预测模型以判断ICU留置导尿管质量控制管理是否可以作为临床上预防尿路感染的独立保护因素。方法回顾性分析2018年1月至2018年12月期间我院ICU收治的185例留置导尿管的患者的资料。根据是否发生尿路感染将患者分为病例组(n=24)和对照组(n=161)。结合性别、年龄、营养状况、高血压、糖尿病、APACHE II评分、留置导尿管时间、导尿管质量控制管理等8项因素进行单因素分析及logistic回归分析。结果单因素结果显示:年龄、营养状况、糖尿病、留置导尿管时间、导尿管质量控制管理等因素与ICU留置导尿管的患者发生尿路感染有关(P<0.05)。logistic回归分析结果显示:留置导尿管时间是ICU患者发生尿路感染的独立危险因素(OR=6.018,95% CI:2.147-16.870,P=0.001),而导尿管质量控制管理ICU患者发生尿路感染的独立保护因素(OR=0.206,95% CI:0.070-0.609,P=0.004)。构建的模型的ROC曲线下面积为0.841,回归模型结果可靠。结论建议在ICU全面推行导尿管质量控制管理,同时尽可能缩短患者留置导尿管的时间,以期减少尿路感染的发生率。 Objective Establish a urinary tract infection risk prediction model to determine whether quality control management of ICU indwelling catheters can be used as an independent protective factor for clinical prevention of urinary tract infections.Methods According to the occurrence of urinary tract infection,185 patients with indwelling catheter in ICU from January 2018 to December 2018 were divided into case group (n=24) and control group (n=161) for retrospective analysis.Univariate analysis and logistic regression analysis were performed based on eight factors including gender,age,nutritional status,hypertension,diabetes,APACHE II score,indwelling catheter time,and catheter quality control management.Results Age,nutritional status,diabetes mellitus,indwelling catheter time,quality control and management of catheter were related to urinary tract infection in univariate analysis(P<0.05).Logistic regression analysis showed that indwelling catheter time(OR=6.018,95% CI:2.147-16.870,P=0.001) was an independent risk factor,while catheter quality control management(OR=0.206,95% CI:0.070-0.609,P=0.004) was an independent protective factor for urinary tract infection.The area under the ROC curve of the model was 0.841,and the regression model was reliable.Conclusion It is recommended to fully implement urinary catheter quality control management in the ICU,while minimizing the time for patients to indwell catheters,in order to reduce the incidence of urinary tract infections.
