摘要
目的探究双腔喉罩与气管插管在腹腔镜下胆囊切除术麻醉中的效果。方法选择2017年3月—2019年4月期间在该院接受腹腔镜胆囊切除术治疗的136例患者为研究对象。基于自主自愿原则,均分为参照组和研究组,每组68例。参照组予以气管插管麻醉,研究组则予以双腔喉罩麻醉。比较两组患者就不同时间点心率(HR)、平均动脉压(MAP)、血气分析指标(PaO2、PaCO2、pH)及术后并发症发生情况。结果参照组置管中及移管中HR(77.54±4.39)、(83.94±4.51)次/min,MAP(99.27±8.58)、(119.84±9.26)mmHg指标优于研究组(60.35±4.01)、(69.11±3.62)次/min,(79.47±7.88)、(101.28±9.16)mmHg,差异有统计学意义(t=32.434、26.964、19.038、16.571,P<0.05),其他时段两组两项指标对比差异无统计学意义(P>0.05);且两组气腹前后各血气分析指标对比差异无统计学意义(P>0.05);研究组并发症发生率(2.9%)优于参照组(14.7%),差异有统计学意义(χ2=5.849,P<0.05)。结论双腔喉罩法就应用于腹腔镜下胆囊切除术中具有稳定血气分析指标的显著效果,且能进一步减少并发症的发生。
Objective To explore the effect of double-cavity laryngeal mask and tracheal intubation in anesthesia during laparoscopic cholecystectomy.Methods 136 patients who underwent laparoscopic cholecystectomy in the hospital from March 2017 to April 2019 were selected as the research objects.Based on the principle of autonomy and voluntariness,they are divided into reference group and research group,with 68 cases in each group.The reference group was given endotracheal intubation anesthesia,and the study group was given dual-chamber laryngeal mask anesthesia.The dim sum rate(HR),mean arterial pressure(MAP),blood gas analysis indexes(PaO2,PaCO2,pH)and postoperative complications were compared between the two groups of patients at different times.Results The HR(77.54±4.39)times/min,(83.94±4.51)times/min,MAP(99.27±8.58)mmHg and(119.84±9.26)mmHg indexes in the reference group were better than those in the study group(60.35±4.01)times/min and(69.11±3.62)times/min,(79.47±7.88)mmHg,(101.28±9.16)mmHg,the difference was statistically significant(t=32.434,26.964,19.038,16.571,P<0.05),there was no statistically significant difference in the comparison of the two indicators between the two groups in other periods(P>0.05);and there was no statistically significant difference in blood gas analysis indexes between the two groups before and after pneumoperitoneum(P>0.05);the complication rate of the study group(2.9%)was better than that of the reference group(14.7%).The difference was statistically significant(χ2=5.849,P<0.05).Conclusion The double-chamber laryngeal mask method can be used in laparoscopic cholecystectomy with a significant effect of stabilizing blood gas analysis indicators and can further reduce the occurrence of complications.
