门冬胰岛素和生物合成人胰岛素对妊娠期糖尿病患者血糖水平和胰岛卩细胞功能的影响 被引量:3

摘要 目的研究门冬胰岛素和生物合成人胰岛素对妊娠期糖尿病患者血糖水平和胰岛0细胞功能的影响。方法选择绵阳市中心医院2018年5月至2019年6月收治的妊娠期糖尿病患者62例,按随机数字表法进行分组,每组31例。对照组患者给予生物合成人胰岛素治疗,观察组患者给予门冬胰岛素治疗,两组患者均持续服药至分娩。比较两组患者治疗前后空腹血糖、餐后2 h血糖水平、糖化血红蛋白、胰岛卩细胞功能指标水平;对比两组患者妊娠结局情况。结果与治疗前比,治疗后两组患者餐后2h血糖、空腹血糖水平、糖化血红蛋白均显著降低,观察组显著低于对照组(PV0.05);与治疗前比,治疗后两组患者空腹胰岛素水平、胰岛素抵抗指数均显著降低,观察组显著低于对照组;两组患者胰岛素敏感指数均增加,观察组显著高于对照组(P<0.05);观察组患者剖宫产率、新生儿窒息率均为6.45%,均显著低于对照组的29.03%(均PV0.05)。结论门冬胰岛素作用于妊娠期糖尿病患者可有效调节患者机体血糖水平和空腹胰岛素水平,改善胰岛素抵抗和胰岛素敏感性,促进机体胰岛卩细胞功能的恢复,改善母婴结局。 To study the effects of insulin aspart and biosynthetic human insulin on blood sugar levels and pancreaticβ-cell function in patients with gestational diabetes.Methods 62 patients with gestational diabetes who were admitted to Mianyang Central Hospital from May 2018 to June 2019 were selected and divided according to the random number table method,with 31 cases in each group.Patients in the control group were treated with biosynthetic human insulin,and patients in the observation group were treated with insulin aspart.Both groups continued to take the drug until delivery.The fasting blood glucose,the 2 h postprandial blood glucose level,glycosylated hemoglobin,and isletβcell function levels before and after treatment were compared between the two groups of patients,and the pregnancy outcomes of the two groups were compared.Results Compared with before treatment,after treatment the 2 h postprandial blood glucose,fasting blood glucose level,and glycosylated hemoglobin levels of the two groups significantly decreased,the observation group was significantly lower than the control group(P<0.05);Compared with before treatment,after treatment the fasting insulin level and insulin resistance index of the two groups significantly reduced,the observation group was significantly lower than the control group;The insulin sensitivity index increased,the observation group was significantly higher than the control group(P<0.05);The cesarean section rate and neonatal asphyxia rate of the observation group were 6.45%,which were significantly lower than 29.03%of the control group(P<0.05).Conclusion Insulin aspart acting on patients with gestational diabetes can effectively regulate the blood glucose level and fasting insulin level of the patient,improve insulin resistance and insulin sensitivity,promote the recovery of the body's pancreaticβ-cell function,and increase maternal and infant outcomes.
