摘要
目的探讨淫羊藿总黄酮辅助治疗老年骨质疏松性桡骨远端骨折的临床疗效以及对骨折愈合和关节功能恢复的影响。方法选入医院2016年1月—2018年3月收治的老年骨质疏松性桡骨远端骨折患者84例,随机分为对照组和观察组,每组42例,两组均予以中医手法复位及石膏固定,观察组加服淫羊藿总黄酮。比较两组的治疗效果、骨折愈合时间。采用采用Gartland-Werley评分系统评价两组患者腕关节功能恢复情况。采用EORTC生命质量测定量表QLQ-C30评价两组患者治疗后生活质量。结果治疗后,观察组掌倾角、尺偏角和骨密度明显高于对照组(P<0.05),而骨折愈合时间及桡骨短缩显著低于对照组(P<0.05);观察组腕关节功能优良率显著高于对照组(P<0.05)。治疗后观察组患者QLQ-C30量表中躯体功能、角色功能、情绪功能、认知功能、疲劳、恶心呕吐、疼痛、失眠、食欲下降、便秘、腹泻和生活质量等领域得分均显著高于对照组(P<0.05)。而并发症发生率明显低于对照组(P<0.05)。结论老年骨质疏松性桡骨远端骨折患者辅以淫羊藿总黄酮治疗,可有效缩短骨折愈合时间,促进腕关节功能恢复,改善骨质疏松和生活质量,减少并发症。
Objective To investigate the clinical efficacy of total flavonoids of epimedium in the treatment of senile osteoporotic distal radius fractures and its effect on fracture healing and joint function recovery.Methods Eighty-four cases of senile osteoporotic distal radius fractures admitted to our hospital from January 2016 to March 2018 were randomly divided into control group and observation group,42 cases in each group.The two groups were treated with manual reduction and plaster fixation,while the observation group added epimedium flavonoids.The therapeutic effect and fracture healing time of the two groups were compared.The Gartland-Werley scoring system was used to evaluate the recovery of wrist function in the two groups.EORTC quality of life scale(QLQ-C30)was used to evaluate the quality of life of the two groups after treatment.Results After treatment,the palmar tilt angle,ulnar deviation angle and bone mineral density of the observation group were significantly higher than those of the control group(P<0.05),while the fracture healing time and radius shortening value were significantly lower than those of the control group(P<0.05).The excellent rate of wrist joint function in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the scores of QLQ-C30 in physical function,role function,emotional function,cognitive function,fatigue,nausea and vomiting,pain,insomnia,loss of appetite,constipation,diarrhea and quality of life in the observation group were significantly higher than those in the control group(P<0.05).The complication rate was significantly lower than that in the control group(P<0.05).Conclusion The treatment of senile osteoporotic distal radius fractures with total flavonoids of epimedium can shorten the healing time of fracture,promote wrist function recovery,improve osteoporosis,life quality and reduce complications.
