摘要
目的观察采用依托咪酯联合丙泊酚对老年高血压患者进行全身麻醉诱导后对患者血流动力学的影响。方法选择2017年10月至2019年8月,我院收治的老年高血压患者102例,按照麻醉用药的不同分为A组、B组和C组,每组各34例。其中A组患者采用丙泊酚进行全身麻醉诱导,B组采用依托咪酯进行全身麻醉诱导,C组采用依托咪酯联合丙泊酚进行全身麻醉诱导。结果经药物麻醉诱导后,三组患者的血压均出现下降现象,A组患者血压下降明显,在T1~T5各时间点与B组和C组比较,其差异具有统计学意义(P<0.05),以T1下降最为明显;B组患者SBP、DBP及HR与T1时刻相比,T2~T5时刻出现明显升高现象,其差异具有统计学意义(P<0.05);C组患者T1、T2、T3、T4、T5各时刻点的DBP和HR差异无明显变化,其差异无统计学意义(P>0.05)。三组患者经相应麻醉诱导后,A组患者的HR在T1时刻明显低于B组和C组,其差异具有统计学意义(P<0.05);B组的HR在T2-T5时刻明显高于A组和C组,其差异具有统计学意义(P<0.05)。结论采用依托咪酯联合丙泊酚对老年高血压患者进行全身麻醉诱导,可以有效减少对血流动力学影响,使患者血压处于平稳状态,对保障手术顺利进行具有积极意义。
Objective To observe the effect of etomidate combined with propofol on hemodynamics in elderly patients with hypertension after general anesthesia induction.Methods One hundred and two elderly patients with hypertension admitted to our hospital from October 2017 to August 2019 were selected and divided into group A,group B and group C according to different anesthetic drugs,with 34 cases in each group.Group A received general anesthesia induction with propofol,group B was induced by etomidate,and group C was induced by etomidate combined with propofol.Result After induction of drug anesthesia,the blood pressure of the three groups decreased significantly.The blood pressure of group A decreased significantly.Compared with group B and group C at T1-5,the difference was statistically significant(P<0.05),especially at T1.SBP,DBP and HR in group B were significantly increased at T2-5 compared with T1,and the difference was statistically significant(P<0.05).There was no significant difference in DBP and HR at T1,T2,T3,T4 and T5 in group C(P>0.05).After induction of anesthesia,HR of group A was significantly lower than that of group B and group C at T1(P<0.05);HR of group B at T2-5 was significantly higher than that of group A and group C(P<0.05).Conclusion Etomidate combined with propofol can effectively reduce the impact on hemodynamics and make the blood pressure in patients with stable state,which has positive significance to ensure the smooth operation.
