摘要
利用2005—2014年中国老年健康影响因素跟踪调查数据库,基于安德森医疗服务利用理论,运用Heckman模型,实证分析城市老年人临终医疗费用的影响因素。主要结果:第一,临终医疗总费用和自付医疗费用总体上呈上升趋势,且自付医疗费用上涨幅度小于总费用,自付支出在收入中的占比较大。第二,有配偶、受教育程度越高、家庭人均收入越高、居住在东部、主要由保险支付医疗费用、临终于医院、患慢性病、自理能力越差的老年人临终总医疗费用越高,但年龄越大的老年人产生的临终医疗费用越少。为控制临终医疗费用的增长,应该加强对老年人常见慢性病患病的预防控制;加强对社会医疗保险制度的监管;开展临终关怀教育;尽快建立和完善我国的长期照护制度。
Using the CLHLS(Tracking Survey Database of Health Influencing Factors for the Elderly in China)from 2005 to 2014,and based on Anderson s medical service utilization model,this paper makes an empirical analysis on the influencing factors of end-of-life medical expenses for the elderly in cities by using Heckman model.The main results are as follows:First,the total end-of-life medical expenses and out-of-pocket medical expenses are generally on the rise,and the increase rate of out-of-pocket medical expenses is smaller than the total expenses,and the self-paid expenses account for a large proportion of income.Second,the total end-of-life medical expenses of the elderly with spouses,higher education level,higher per capita income of families,living in the eastern region,mainly paying for medical expenses by insurance,dying at the hospital,poor self-care ability and chronic diseases are higher.But the elderly with older age have lower expenses at the end of life.In order to control the increase of dying medical expenses,we should strengthen the prevention and control of common chronic diseases of the elderly;strengthen the supervision of the social medical insurance system;carry out hospice care education;establish and improve our long-term care system as soon as possible.
