摘要
桥本甲状腺炎(Hashimoto thyroiditis, HT)是一种自身免疫性甲状腺疾病,中医将其归属于"瘿"病的范畴,其病情若不进行干预治疗,会呈现特定的规律,早期无症状,中期甲状腺功能亢进期,晚期甲状腺功能减退期,最后发展为永久性甲状腺功能减退或甲状腺癌。目前,西药治疗存在一定局限性,而中医治疗瘿病已有千年历史。因此,本研究基于中医"治未病"思想,分析HT的临床分期以及各期的治未病思想的应用,并探究逍遥散加味颗粒结合"治未病"思想治疗HT的作用机制。根据"治未病"思想,治疗HT应在未病时要未病先防,在疾病的早期应积极运用中药治疗,尽可能治愈或延缓病情发展,在中期和晚期治疗时不仅治已病,还要阻断病情,并治愈后防止疾病复发;逍遥散加味颗粒结合"治未病"思想成方,内含有大量的补益药,帮助机体扶助正气,增强免疫功能,从而对HT的治疗和延缓病情产生作用。
Hashimoto thyroiditis(HT) is an autoimmune thyroid disease. Traditional Chinese medicine(TCM) classifies it as a type of "goiter" disease. If there is no intervention, its condition will show a specific pattern. With Early Asymptomatic, intermediate hyperthyroidism, advanced hypothyroidism, it finally progresses to permanent hypothyroidism or thyroid cancer. At present, Western medicine treatment has certain limitations, and TCM has been treating goiter disease for thousands of years. Therefore, this paper analyzes the clinical stages of HT and the application of the thoughts of treating the disease based on the idea of "treating the disease before it arising" of TCM, and explores the mechanism of modified Xiaoyao Powder Granules combined with the idea of "treating the disease before it arising" in treating HT. According to the idea of "treating the disease before it arising", treatment of HT should be prevented before the disease arises. In the early stages of the disease, Chinese medicine should be actively used to cure or delay the development of the disease as much as possible. In the middle and late stages of treatment, we should not only prevent the recurrence of the disease but also prevent disease recurrence after cure. Modified Xiaoyao Powder Granules combined with the idea of "treating the disease before it arising" contains a large number of tonics to help the body support vital qi, enhance the immune function, Thereby it having an effect on the treatment and delay of HT.
