冠状动脉旁路移植术后静脉桥血管狭窄的危险因素分析 被引量:2

摘要 目的探究冠状动脉旁路移植术(CABG)后静脉桥血管狭窄的影响因素。方法选择2016年9月至2018年10月于我院就诊的145例CABG术后再发胸痛的患者纳入研究,所有患者均行冠状动脉造影检查明确静脉移植血管(SVG)病变情况,比较SVG病变组及非SVG病变组患者临床资料,分析CABG术后SVG狭窄的危险因素。结果入组145例患者共261支桥血管中,共计102例患者发生SVG病变,病变涉及122支SVG桥血管;SVG病变组患者年龄、搭桥年龄显著高于非SVG病变组(P<0.05),合并高脂血症比例显著高于非SVG病变组(P<0.05);SVG病变组患者血小板计数、红细胞压积、中性粒细胞/淋巴细胞比率显著高于非SVG病变组(P<0.05);多因素分析示,搭桥数目>2支、年龄>65岁、伴有高脂血症是CABG术后发生SVG病变的独立危险因素(P<0.05)。结论CABG术后极易发生SVG病变,高危风险因素患者宜定期检查,提高对SVG病变的防范意识。 Objective To investigate the influencing factors of saphenous vein graft(SVG)stenosis after coronary artery bypass grafting(CABG).Methods A total of 145 patients with relapse of chest pain after CABG admitted in our hospital between September 2016 and October 2018 were included in this study.All patients underwent coronary angiography for confirmation of SVG stenosis.The clinical data of patients with and without SVG lesion were compared.Risk factors of SVG stenosis after CABG were investigated.Results Among the 145 patients with 261 SVGs,102 patients developed SVG stensosis involving 122 SVGs.The patient age and the age at bypass surgery were significantly higher in patients with SVG stenosis than in those without(P<0.05).There was a significantly higher proportion of comorbidity with hyperlipidemia in patients with SVG stenosis than in those without(P<0.05).The patients with SVG stenosis were more likely to have significantly higher platelet count,hematocrit,and neutrophil-to-lymphocyte ratio,compared with those without(P<0.05).Multivariate analysis showed that the>2 bypasses,age over 65 years,and comorbidity with hyperlipidemia were independent risk factors for SVG stenosis after CABG(P<0.05).Conclusion SVG stenosis is readily prone to happen after CABG.Patients at high risks should be on regular checkup and higher awareness for prevention of SVG stenosis.
