摘要 目的了解pH值对少、弱精子症和无精症男性精液质量的影响作用。方法以2015—2019年门诊3315例不育男性患者为研究对象,以一般线性模型统计分析少、弱精子症、无精症、严重少精症和精液参数正常男性的精液pH值差异及pH值与精子活力、浓度、存活率、精子形态间的相关性。结果在无精、严重少精、少精、弱精、少弱精和精子活力与总数正常的患者中测得的pH值差异有统计学意义(P<0.01),严重少精症患者的精液pH值显著低于其他组,少、弱精和无精症组显著低于正常组。pH值与前向运动精子、存活率、精子形态正常率呈正相关(P<0.01)。在pH值为7.2时的精液质量参数显著高于其他pH值时测得的数值(P<0.01),pH值为6.4时参数值最低。结论少、弱精子症可能与pH值偏酸性有关,而且无精子症和弱精子症受pH值酸性影响更为显著。pH值对精子活力、存活率和精子形态均有显著影响,发现精液酸性异常的原因并改善精液的酸碱环境,对不育男性的临床诊断与治疗有重要意义。 Objective To investigate the impact of pH value on the semen quality of men with oligospermia,asthenospermia and azoospermia.Methods Included in this study were 3315 infertile males who visited our outpatient clinics between 2015 and 2019.By using a general linear model,the differences in semen pH value among men with oligospermia,asthenospermia,azoospermia,severe oligospermia,and normospermia,as well as the correlation between semen pH value and sperm motility,concentration,viability and morphology were statistically analyzed.Results There were significant statistical differences in the measured pH values among patients with azoospermia,severe oligospermia,oligospermia,asthenospermia,oligo-asthenospermia and normospermia(P<0.01).The semen pH values were significantly lower in severe oligospermic patients than in other groups,and so were in oligospermic,asthenospermic and azoospermic males than in normospermic males.The semen pH value was significantly positively correlated with the rates of sperm with progressive motility,viability and normal morphology(P<0.01).The semen quality parameters was significantly higher when measured at pH=7.2 compared with those measured at other pH values(P<0.01),and were lowest when measured at pH=6.4.Conclusion Oligospermia and asthenospermia may be associated with acidic semen pH.The impacts of acidic semen pH value can be more significant in azoospermia and asthenospermia.The semen pH value affects sperm motility,viability and morphology.Identifying the cause of the abnormal acidity and improving the acid-base environment of semen is of great significance to the clinical diagnosis and treatment of male infertility.
