摘要
目的研究内皮细胞特异性分子-1(ESM-1)在急性心肌梗死(AMI)患者血浆中的表达及其临床意义。方法选择开封市中心医院2015年3月-2018年3月期间收治的124例AMI患者作为AMI组,另取同期进行体检的健康志愿者80人作为对照组。评价AMI患者的病情及预后,检测血浆中ESM-1、磷酸肌酸激酶同工酶(CKMB)、低密度脂蛋白胆固醇(LDL-C)、C反应蛋白(CRP)水平。结果 AMI组患者血浆ESM-1、CK-MB、LDL-C、CRP水平均明显高于对照组(P<0.05);AMI组中三支病变患者血浆ESM-1、CK-MB、LDL-C、CRP水平均明显高于单支病变和二支病变患者(P<0.05),Gensini积分>32分患者血浆ESM-1、CK-MB、LDL-C、CRP水平均明显高于Gensini积分20~32分和<20分患者(P<0.05),发生主要不良心脑血管事件(MACCE)患者血浆ESM-1水平明显高于未发生MACCE患者(P<0.05),CK-MB、LDL-C、CRP水平与未发生MACCE患者比较差异无统计学意义(P>0.05);AMI组患者血浆ESM-1水平与CK-MB、LDL-C、CRP水平均呈正相关关系。结论血浆ESM-1与AMI的病情及预后有关,可能成为判断AMI病情、预测AMI预后的指标。
Objective To study the expression of endothelial specific molecule-1(ESM-1) in plasma of patients with acute myocardial infarction(AMI) and its clinical significance.Methods Patients with AMI admitted to our hospital from March2015 to March 2018 were selected as AMI group and healthy volunteers for physical examination as control group.The condition and prognosis of patients with AMI were evaluated.The plasma levels of ESM-1,creatine kinase isoenzymes(CKMB),low-density lipoprotein cholesterol(LDL-C) and C-reactionprotein(CRP) were measured.Results The plasma ESM-1,CK-MB,LDL-C and CRP levels in patients with AMI were significantly higher than those in control group(P<0.05).The plasma ESM-1,CK-MB,LDL-C and CRP levels in patients with three vessel lesions in AMI group were significantly higher than those in patients with single vessel lesion and two vessel lesions(P<0.05).The plasma ESM-1,CK-MB,LDL-C and CRP levels in patients with Gensini score>32 points in AMI group were significantly higher than those with Gensini score 20-32 points and <20 points(P<0.05).The plasma ESM-1,CK-MB,LDL-C and CRP levels in patients with major adverse cardiovascular events(MACCE) in AMI group were significantly higher than those in patients without MACCE(P<0.05).The ESM-1 levels of patients in AMI group were positively correlated with CK-MB,LDL-C and CRP levels.Conclusion Plasma ESM-1 was related to the condition and prognosis of AMI,which might be an indicator to judge the condition of AMI and predict the prognosis of AMI.
