摘要
目的探讨血清载脂蛋白B与载脂蛋白A1比值(ApoB/ApoA1)与冠状动脉狭窄程度的关系,评价其在冠心病(CHD)中的诊断价值.方法选取2004-2017年我院首次行冠状动脉造影检查并符合入组条件的患者198例,分为CHD组(130例)、非CHD组(68例),再根据Gensini(GS)积分评估价病变程度,对各血脂常规指标行ROC曲线分析及相关分析.结果①CHD组患者较非CHD组ApoB/ApoA1水平增高(1.38±0.51比0.64±0.35,P<0.01).②多因素Logistic回归分析显示,ApoB/ApoA1为CHD独立危险因素(P<0.01),ApoA1为CHD的保护因素(P<0.01).③线性相关分析显示,ApoB/ApoA1(r=0.742)与GS呈正相关(P<0.01),ApoA1(r=-0.773)与GS呈负相关(P<0.01).④ROC曲线分析ApoB/ApoA1(曲线下面积AUC 0.836;敏感度0.853;特异度0.574;P<0.01)最佳诊断界值(cut-off值)为0.678.结论①ApoB/ApoA1为CHD的独立危险因素;ApoA1为CHD的保护因素;②ApoB/ApoA1能有效反映冠状动脉狭窄程度.
Objective To investigate the relationship between the serum ApoB/ApoAl ratio and the degree of coronary artery stenosis and to evaluate its diagnostic value in coronary heart disease(CHD).Methods 198 patients who first underwent coronary angiography were divided into CHD group(n=130)and non-CHD group(n=68).The degree of lesion was evaluated according to Gensini score(GS)and the routine indexes of blood lipids were analyzed by ROC curve and correlation analysis.Results(1)The level of ApoB/ApoAl in CHD group was higher than that in non-CHD group(1.38±0.51 vs.0.64±0.35,PvO.Ol).(2)Multivariate Logistic regression analysis showed that ApoB/ApoAl was an independent risk factor for CHD(P<0.01)and ApoAl was a protective factor for CHD(P<0.01).(3)Linear correlation analysis showed that there was a positive correlation between ApoB/ApoA 1 and GS(P<0.01)and a negative correlation between ApoAl and GS(P<0.01).(4)ROC curve analysis ApoB/ApoAl(the area under the curve AUC was 0.836;sensitivity was 0.853;specificity was 0.574;P<0.01)showed the best diagnostic cut-off point(cutoff value)was 0.67&Conclusion(l)ApoB/ApoAl is an independent risk factor for CHD and ApoAl is a protective factor for CHD;(2)ApoB/ApoAl can effectively reflect the degree of coronary artery stenosis.
