摘要
目的探讨磁共振弥散加权成像(DWI)表观扩散系数(ADC)值在慢性肾脏疾病诊断中的应用。方法选取2016年1月~2018年12月在大连市友谊医院肾内科住院的20例慢性肾脏疾病患者(病例组)及32例肾脏功能正常的健康人(对照组)行肾脏磁共振检查,除常规磁共振平扫检查外进行DWI扫描,比较分析两组肾脏ADC值间的差异,分析肾脏ADC值与血尿素氮(BUN)水平之间的关系。结果对照组的肾脏ADC值为(2.80±0.12)×10-3 mm2/s,病例组的肾脏ADC值为(2.15±0.22)×10-3 mm2/s;病例组的肾脏ADC值低于对照组,差异有统计学意义(P<0.05)。慢性肾脏疾病患者肾脏ADC值与血尿素氮水平成负相关(r=-0.53,P=0.001)。结论肾脏DWI成像ADC值可用于慢性肾脏疾病的诊断,可以作为肾功能损害评价的指标之一,为临床治疗提供更有效的影像学诊断支持。
Objective To explore the application of apparent diffusion coefficient(ADC)value using MRI diffusion-weighted imaging(DWI)in the diagnosis of chronic kidney disease.Methods Twenty patients with chronic kidney disease(case group)who were hospitalized in the Nephrology Department of Dalian Friendship Hospital and 32 healthy participants with normal renal function(control group)from January 2016 to December 2018 were selected for renal magnetic resonance examination.In addition to the conventional magnetic resonance scan,DWI scan was performed to compare and analyze the difference in ADC values of the two groups and the relationship between the kidney ADC value and blood urea nitrogen(BUN)level.Results The kidney ADC value of the control group was(2.80±0.12)×10-3 mm2/s,and the kidney ADC value of the case group was(2.15±0.22)×10-3 mm2/s.The kidney ADC value of the case group was lower than that of the control group,the difference was statistically significant(P<0.05).The ADC value of kidney in patients with chronic kidney disease was negatively correlated with the level of BUN(r=-0.53,P=0.001).Conclusion The ADC value of kidney DWI imaging can be used for the diagnosis of chronic kidney disease and can be used as one of the indicators for the evaluation of renal damage,which provides effective imaging diagnosis support for clinical treatment.
