摘要
目的探讨茯苓-桂枝药对对慢性心力衰竭大鼠心肌组织的影响及可能作用机制。方法60只雄性Wistar大鼠随机分为假手术组、模型组、卡托普利组及茯苓桂枝高、中、低剂量组,每组10只。除假手术组外,其余各组采用腹主动脉缩窄的方法制备慢性心力衰竭大鼠模型。造模成功后假手术组与模型组给予2ml/kg蒸馏水灌胃,茯苓桂枝高剂量组给予茯苓2.16g/(kg·d)、桂枝1.62g/(kg·d)灌胃,茯苓桂枝中剂量组给予茯苓1.08g/(kg·d)、桂枝0.81g/(kg·d)灌胃,茯苓桂枝低剂量组给予茯苓0.54g/(kg·d)、桂枝0.41g/(kg·d)灌胃,卡托普利组给予卡托普利混悬溶液6.3mg/(kg·d)灌胃,均每日1次,共4周。比较各组大鼠收缩压、心脏质量指数(HMI),检测血清中血管紧张素Ⅱ(AngⅡ)、肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、内皮素1(ET-1)含量,HE染色观察心肌组织病理改变,检测心肌组织血管紧张素Ⅱ受体1(AT1)含量。结果与假手术组比较,模型组大鼠HMI、收缩压、心肌组织AT1及血清AngⅡ、ET-1、TNF-α、IL-1β水平升高(P<0.01),病理显示心肌细胞束的间隙稍增宽,心肌纤维排列紊乱、变粗肥大。与模型组比较,卡托普利组及茯苓桂枝中、高剂量组大鼠HMI、收缩压、心肌组织AT1及血清AngⅡ、ET-1、TNF-α、IL-1β水平降低(P<0.05或P<0.01);茯苓桂枝高剂量组与卡托普利组病理显示心肌组织炎症细胞减少,细胞形态较清晰,心肌纤维化程度低。与卡托普利组比较,茯苓桂枝高剂量组各指标差异无统计学意义(P>0.05)。结论茯苓-桂枝药对可以调节慢性心力衰竭大鼠血压,抑制心肌肥厚与心室重构,改善大鼠心肌纤维化,可能与控制炎症反应、抑制神经内分泌过度激活有关,且以高剂量效果最佳。
Objective To study the effects and mechanisms of Poria and Guizhi(Ramulus Cinnamomi)drug-pair on myocardial tissue of rats with chronic heart failure(CHF).Method A total of 60 male Wistar rats were randomly divided into a sham operation group,a model group,a captopril group and drug-pair high,medium and low dose groups,with 10 rats in each group.Except for the sham operation group,method of constricting the abdominal aorta was used to prepare the model of chronic heart failure(CHF).After successful modeling,the sham operation group and the model group were given distilled water 2 ml/kg by gavage,and the drug-pair high-dose group was given Poria 2.16 g/(kg·d)and Ramulus Cinnamomi 1.62 g/(kg·d)by gavage;the drug-pair medium-dose group was given Poria 1.08 g/(kg·d),Ramulus Cinnamomi 0.81 g/(kg·d);and the drug-pair low-dose group was given Poria 0.54 g/(kg·d)and Ramulus Cinnamomi 0.41 g/(kg·d);the captopril group was given captopril suspension 6.3 mg/(kg·d)by gavage.All groups were given once a day for 4 weeks.The systolic blood pressure and heart mass index(HMI)of rats in each group were compared.The serum angiotensinⅡ(AngⅡ),tumor necrosis factorα(TNF-α),interleukin 1β(IL-1β)and endothelin(ET-1)content were detected.HE staining was used to observe the pathological changes of myocardial tissue,and detect the content of angiotensinⅡreceptor 1(AT1)in myocardial tissue.Results Compared with the sham operation group,the model group had significantly higher levels of HMI,systolic blood pressure,myocardial tissue AT1,serum AngⅡ,ET-1,TNF-α,and IL-1β(P<0.01),and pathology showed that the gaps of myocardial cell bundles were slightly widened,and the arrangement of myocardial fibers was disordered,thickened and hypertrophy.Compared with the model group,the levels of HMI,systolic blood pressure,myocardial tissue AT1,and serum AngⅡ,ET-1,TNF-α,and IL-1βin captopril group and drug-pair high-dose groups were significantly reduced(P<0.05 or P<0.01);the pathology of drug-pair high-dose group and captopril group had reduced myocardial tissue inflammatory cells,clear cell morphology,and low degree of myocardial fibrosis.Compared with the captopril group,there was no statistically significant difference in the drug-pair high-dose group(P>0.05).Conclusion Drug-pair Poria and Ramulus Cinnamomi could regulate blood pressure in CHF rats,inhibit myocardial hypertrophy and ventricular remodeling,and improve myocardial fibrosis in rats.It may be related to controlling inflammation,and inhibiting neuroendocrine overactivation,and it has best efficacy in high doses.
