摘要
目的:探讨内镜下喷洒凝血酶在治疗经皮冠状动脉介入术(PCI)后抗血小板药物所致上消化道出血中的效果。方法:选择2017年1月~2019年1月于本院就诊的120例行PCI后抗血小板药物所致上消化道出血病患者作为研究对象,将所有患者随机分为两组。对照组患者(60例)采用奥美拉唑治疗,试验组患者(60例)实施内镜下喷洒凝血酶治疗。比较两组患者止血时间、临床疗效及支架内血栓形成发生率、血小板、凝血功能指标水平的变化。结果:试验组患者治疗后总有效率(98.33%)明显高于对照组(86.67%),差异具有统计学意义(P<0.05)。对照组患者治疗后平均止血时间(2.35±0.42)d明显高于试验组止血时间(3.84±0.55)d,差异具有统计学意义(t=16.678,P<0.001)。结论:内镜下喷洒凝血酶在治疗冠心病患者PCI术后抗血小板药物所致的上消化道出血效果显著,同时不增加支架内血栓形成率及心血管不良事件发生。
Objective:To investigate the effect of endoscopic thrombin spray in the treatment of upper gastrointestinal hemorrhage caused by antiplatelet drugs after percutaneous coronary intervention(PCI).Methods:From January 2017 to January 2019,120 patients with upper gastrointestinal hemorrhage caused by antiplatelet drugs after pic were selected as the study objects,and all patients were randomly divided into two groups.Patients in the control group(60 cases)were treated with omeprazole and patients in the test group(60 cases)were treated with thrombin spray under endoscope.The hemostasis time,clinical effect,the incidence of thrombosis,the changes of platelet and coagulation function were compared between the two groups.Results:The total effective rate of the experimental group(98.33%)was significantly higher than that of the control group(86.67%),the difference was statistically significant(P<0.05).The average hemostasis time of the control group was(2.35±0.42)d,which was significantly higher than that of the test group(3.84±0.55)d,the difference was statistically significant(t=16.678,P<0.001).Conclusion:Endoscopic thrombin spray is effective in the treatment of upper gastrointestinal bleeding caused by antiplatelet drugs in patients with coronary heart disease after pic,and does not increase the rate of stent thrombosis and cardiovascular adverse events.
