摘要
目的探讨不同脑叶癫痫脑电图异常放电特点的差异,为癫痫的精准防治提供新的理论依据。方法收集2012年1月1日至2017年1月1日山东第一医科大学第二附属医院癫痫门诊确诊为癫痫并伴有异常放电的患者资料进行回顾性分析。共505例癫痫患者,其中额叶癫痫(frontal lobe epilepsy, FLE)162例,颞叶癫痫(temporal lobe epilepsy, TLE)223例,枕叶癫痫(occipital lobe epilepsy, OLE)75例,顶叶癫痫(parietal lobe epilepsies,PLE)45例。对癫痫患者发作期及发作间期脑电图进行分析,总结不同脑叶癫痫脑电图异常放电特点的差异。结果 FLE异常放电以额区阵发棘、尖波为主,TLE以单侧或双侧颞叶偶发棘波为主,OLE以枕区偶发高幅棘波、尖波为主,PLE以顶区偶发异常波形多见。结论不同脑叶癫痫的异常放电各有特点。
Objective:To investigate the differences of different lobe epilepsy in discharge characteristics and to provide a new theoretical basis for the prevention and treatment of epilepsy.Methods:The data of patients who were diagnosed with epilepsy and accompanied by abnormal discharge from the epilepsy clinic of the Second Affiliated Hospital of Shandong First Medical University from January 1,2012 to January 1,2017 were collected for retrospective analysis.A total of 505 patients with epilepsy,including 162 cases of FLE,223 cases of TLE,75 cases of OLE and 45 cases of PLE.EEG was used to analyze the onset and duration of EEG and abnormal discharge characteristics of frontal lobe epilepsy,temporal lobe epilepsy,occipital lobe epilepsy,parietal lobe epilepsy and insular epilepsy.The differences of different lobe epilepsies in EEG were summarized.Results:In FLE,the abnormal discharge in frontal area was mainly paroxysmal spike and sharp waves;In TLE,abnormal discharge in the temporal region with unilateral or bilateral accidental was the spike waves;In OLE,the abnormal discharge in the occipital area with occasional high amplitude was spike and sharp waves;In PLE,abnormal discharge was mainly abnormal wave in the top region.Conclusion:Different lobe epilepsies in discharge characteristics have their own differences,which provides theoretical basis for the prevention and treatment of epilepsy.
