摘要
目的探究武汉汉族人群缺血性脑卒中患者阿司匹林抵抗的性别差异现象及其可能分子机制。方法选取武汉地区汉族人群1103例,其中健康筛查受试者897例为对照组和缺血性脑卒中患者206例为观察组。检测入选者环氧化酶1(COX-1)A-842G、血小板膜糖蛋白Ⅱb/Ⅲa(GPⅡb/Ⅲa)PIA1/A2及血小板内皮细胞聚集受体1(PEAR1)rs12041331位点基因型,测定凝血指标,包括纤维蛋白原、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间及国际标准化比值(INR)。结果1103例入选者中,COX-1 A-842G位点基因型AA及GPⅢa PIA2 TT基因型均占100%,PEAR1 rs12041331位点野生型(GG)及突变型(AA型+AG型)分别为401例(36.36%)、702例(63.64%)。观察组PEAR1基因突变型明显高于对照组(71.84%vs 61.76%,P<0.05)。观察组男性PEAR1 rs12041331突变率明显高于女性(77.05%vs 64.29%,P<0.05),男性PEAR1突变型患者APTT、PT及INR明显低于野生型患者(P<0.05,P<0.01)。结论汉族人群中缺血性脑卒中男性PEAR1 rs12041331突变患者阿司匹林治疗下更容易出现凝血倾向。PEAR1 rs12041331可能是缺血性脑卒中患者阿司匹林抵抗性别差异的分子标志物。
Objective To study the gender difference in rs12041331-induced aspirin resistance(AR)and its possible molecular mechanisms in Wuhan Han ischemic stroke(IS)patients.Methods A total of 1103 Han people in Wuhan area were divided into control group(n=897)and IS group(n=206).Their genotypes at the sites of COX-1(A-842 G),GPⅡb/Ⅲa PIA1/A2 and PEAR1-rs12041331 were detected.The APTT,PT and INR were recorded.Results The genotype(AA)at COX-1(A-842 G)and the genotype(TT)at GPⅢa(PIA2)accounted for 100%.The wild type homozygous genotye(GG)and mutant genotypes(AA and AG)at PEAR1-rs12041331 were detected in 401 people(36.36%)and 702(63.64%)people respectively.The mutation rate of PEAR1 was significantly higher in IS group than in control group(71.84%vs 61.76%,P<0.05).The mutation rate of PEAR1-rs12041331 was significantly higher in males than females of IS group(77.05%vs 64.29%,P<0.05).The APTT and PT were significantly shorter and the INR was significantly lower in male patients with PEAR1-rs12041331 mutation than in those with PEAR1-rs12041331 wild type(P<0.05,P<0.01).Conclusion Coagulation is easy to occur in Wuhan Han male IS patients with PEAR1-rs12041331 mutation.PEAR1-rs12041331 may become a molecular marker of gender difference in AR of IS patients.
