摘要
目的比较不同入路腹腔镜右半结肠癌根治术治疗结肠癌的临床效果及安全性。方法将114例结肠癌患者根据随机数表法分为观察组和对照组,各57例。观察组采用尾侧中间联合入路腹腔镜右半结肠癌根治术治疗,对照组采用传统中间入路腹腔镜右半结肠癌根治术治疗。比较两组患者的围术期情况、炎症反应指标、复发率、存活率及术后并发症发生情况。结果术前及术后1、5 d,两组的WBC、CRP及IL-6水平比较,差异无统计学意义(P>0.05);术后1 d,两组的WBC、CRP及IL-6水平均高于术前及术后5 d,且术后5 d高于术前(P<0.05)。两组的手术时间、术中出血量、淋巴结清扫数量、术后排气时间、进食流质时间、术后住院时间,随访1、2年的复发率及存活率、术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论与传统中间入路相比,尾侧中间联合入路腹腔镜右半结肠癌根治术治疗结肠癌的近远期疗效相当,安全性高。
Objective To compare the clinical effect and safety of laparoscopic radical right hemicolonectomy with different approaches in the treatment of colon cancer.Methods A total of 114 patients with colon cancer were divided into observation group and control group by random number table method,with 57 cases in each group.The observation group was treated with laparoscopic radical right hemicolonectomy via caudal middle combined approach,while the control group was treated with laparoscopic radical right hemicolonectomy via traditional middle approach.The perioperative situation,inflammatory response indexes,recurrence rate,survival rate and postoperative complications of the two groups were compared.Results Before operation and at 1 and 5 d after operation,there were no significant differences in WBC,the levels of CRP and IL-6 between the two groups(P>0.05);at 1 d after operation,WBC,the levels of CRP and IL-6 in the two groups were higher than those before operation and at 5 d after operation,and those at 5 d after operation were higher than those before operation(P<0.05).There were no significant differences in operation time,intraoperative blood loss,number of lymph node dissection,postoperative exhaust time,fluid intake time,postoperative hospital stay,recurrence rate,survival rate after 1 and 2 years of follow-up and total postoperative complication rate of the two groups(P>0.05).Conclusion Compared with the traditional middle approach,the laparoscopic radical right hemicolonectomy via caudal-middle combined approach in the treatment of colon cancer has similar short-term and long-term efficacy,with high safety.
