摘要
目的:通过分析新型冠状病毒肺炎患者(COVID-19)不同临床分型及病程不同阶段的C反应蛋白(CRP)动态变化特征,以探讨动态监测C反应蛋白对COVID-19病情预估的作用。方法:回顾性分析2020年1月12日—3月30日湖北省安陆市人民医院收治的116例COVID-19患者的临床资料,根据患者的临床特点按照中国第5版COVID-19诊治指南标准分组,其中轻型组0例、普通型组71例、重型组30例、危重型组15例,分析不同临床分型COVID-19患者的血清CRP动态变化特点与病情预后的相关性。结果:116例COVID-19患者在病毒感染初期普遍表现为CRP水平升高,且CRP升高程度与COVID-19疾病严重程度有关;非危重症患者在治疗1周后CRP明显下降或接近正常水平,危重症患者的CRP下降出现在治疗2~3周后;幸存结局患者的CRP在1周左右可迅速下降至接近正常范围,死亡结局患者经治疗后CRP水平持续波动在高于正常范围水平。结论:动态检测COVID-19患者CRP水平变化有助于判断病情变化、严重程度及预后。
Objective:To investigate the effect of dynamic monitoring C-reative protein(CRP)on Corona Virus Diseases 2019(COVID-19)condition prediction by analyzing the dynamic changes of CRP in different clinical classification and stages of disease in patients with COVID-19.Methods:The data of patients with COVID-19 were collected in the Anlu People’s Hospital,Hubei Province from January 12th to March 30th,2020.The cases were grouped by the fifth edition guideline for COVID-19 diagnosis and treatment of China according to the clinical characteristics of the patient,including light group 0 case,regular group 71 cases,severe group 30 cases,critically ill group 15 cases.A retrospective study was performed to analyze the dynamic changes of serum CRP in patients with different clinical classifications of COVID-19 and the correlation with disease prognosis.Results:CRP was generally elevated in the early stage of virus infection in 116 patients with COVID-19,and the degree of elevation were related to the severity of disease;CRP in non-critical patients could be significantly reduced or approached normal levels after 1 week of treatment,and 2~3 weeks for critically ill patients;CRP could quickly drop to almost normal level about 1 week in patients with surviving outcomes,in the patients with death outcomes,CRP continued to fluctuate above the normal range after treatment.Conclusion:Detecting changes in CRP over time dynamically in patients with COVID-19 contribute to assess disease status,disease severity and prognosis.
