摘要
目的探讨血清淀粉样蛋白A(SAA)及降钙素原(PCT)水平在儿童感染性疾病中的诊断价值。方法选择2019年7-12月该院儿科住院的儿童感染性疾病患者160例,分为细菌感染组85例和病毒感染组75例,并选取同期体检健康儿童65例为健康对照组,检测各组间血清中SAA、PCT、C反应蛋白(CRP)、白细胞计数(WBC)及淋巴细胞计数(LYMPH)的水平,利用受试者工作特征曲线评价SAA、PCT、CRP、WBC及LYMPH对儿童感染性疾病的诊断价值。结果与健康对照组比较,细菌感染组急性期SAA、PCT、CRP水平及WBC升高(P<0.05),而病毒感染组急性期SAA、LYMPH升高(P<0.05)。细菌感染组SAA和PCT的mRNA表达水平较病毒感染组和健康对照组升高(P<0.05)。SAA、PCT、CRP、WBC对儿童细菌感染性疾病具有诊断价值(P<0.05),而SAA、LYMPH对儿童病毒感染性疾病具有诊断价值(P<0.05)。结论SAA水平在儿童细菌和病毒感染时均升高,而PCT水平在细菌感染时升高,动态监测血清SAA、PCT水平变化,对临床指导用药具有重要的临床意义。
Objective To investigate the diagnostic value of serum amyloid A(SAA)and procalcitonin(PCT)levels in children with infectious diseases.Methods A total of 160 children with infectious diseases hospitalized in pediatrics from July to December 2019 were selected and divided into bacterial infection group(85 cases)and virus infection group(75 cases),and 65 healthy children were selected as the the healthy control group.The serum levels of SAA,PCT,C-reactive protein(CRP),white blood cell count(WBC)and lymphocyte count(LYMPH)were measured,the receiver operating characteristic curve was used to evaluate the diagnostic value of SAA,PCT,CRP,WBC and LYMPH in children with infectious diseases.Results Compared with the healthy control group,the levels of SAA,PCT,CRP and WBC in the acute phase of bacterial infection group were increased(P<0.05),while SAA and LYMPH in the acute phase of virus infection group were increased(P<0.05).The mRNA expression levels of SAA and PCT in bacterial infection group were higher than those in virus infection group and healthy control group(P<0.05).SAA,PCT,CRP and WBC had diagnostic value for bacterial infectious diseases in children(P<0.05),while SAA and LYMPH had diagnostic value for viral infectious diseases in children(P<0.05).Conclusion SAA levels increase in children with bacterial and viral infection,while PCT levels increase during bacterial infection.Dynamic monitoring of changes in serum SAA and PCT levels has important clinical significance for clinical guidance of medication.
