摘要 假体周围感染(PJI)是人工关节置换术后灾难性的并发症,其诊疗一直是关节外科领域的研究热点和难点。目前,PJI的治疗策略包括抗生素治疗、保留假体清创灌洗术、Ⅰ期翻修术、Ⅱ期翻修术、关节融合术、截肢以及生物治疗等。常规治疗不能取得满意的疗效,而生物治疗作为一种新兴的治疗模式,在PJI治疗中具有独特的优势。该文就PJI的危险因素和感染源、诊断和分型,以及治疗策略进行综述,为临床PJI治疗方式的选择提供参考。 Periprosthetic joint infection(PJI)is a catastrophic complication after artificial joint replacement,and its diagnosis and treatment has always been a great challenge in the field of orthopedics.At present,the treatment strategies for PJI include suppressive antibiotic therapy,debridement antibiotics irrigation of the retained,prosthesis,one-stage revision,twostage revision,arthrodesis,amputation,and biotherapy,etc.Conventional treatment can not achieve satisfactory results.As a new treatment mode,biotherapy has unique advantages in PJI treatment.This article reviews the risk factors and the source of infection,diagnosis,classification and treatment strategies of PJI,in order to provide valuable reference for clinical treatment of PJI.
