新型箱式给氧改善新生儿呼吸窘迫综合征患儿血清CRP、TGF-β1和MMP-9水平及氧供 被引量:9

Effect of New Box Oxygen on the Expression of CRP,TGF-β1 and MMP-9 and the Improvement of Oxygen Supply in Neonatal Respiratory Distress Syndrome

摘要 目的:探讨新型箱式给氧对新生儿呼吸窘迫综合征(NRDS)患儿血清C-反应蛋白(CRP)、转化生长因子-β1(TGF-β1)、基质金属蛋白酶-9(MMP-9)的水平及氧供改善的影响。方法:选取2017-06-2020-06本院收治的NRDS患儿87例为研究对象,按随机数字表法将其分为观察组(n=45)和对照组(n=42),观察组患儿予以新型箱式给氧进行治疗,对照组患儿予以头罩吸氧进行治疗。治疗前及氧疗完成后检查血清指标,对比两组临床治疗效果、血清CRP、TGF-β1、MMP-9、心率、呼吸、氧合指数、血气分析指标pH值、二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)和氧饱和度(SaO2)及总不良反应发生率。结果:治疗后,观察组NRDS患儿临床总有效率明显高于对照组(88.89%vs71.43%,P<0.05);观察组NRDS患儿血清CRP、TGF-β1和MMP-9水平以及心率和呼吸频率明显低于对照组(P<0.05);而氧合指数、pH值、PaO2和SaO2均明显高于对照组(P<0.05);两组NRDS患儿PaCO2对比无明显差异(P>0.05)。结论:新型箱式给氧是一种安全有效的给氧方法,能有效降低NRDS患儿炎症状态,改善心率、呼吸、氧合指数及血气指标,有助于提高临床疗效。 Objective:To investigate the effects of new box-type oxygen administration on serum C-reactive protein(CRP),serum transforming growth factor-β1(TGF-β1),serum matrix metalloproteinase 9(MMP-9)expression and oxygen supply improvement in neonatal respiratory distress syndrome(NRDS)children.Method:Eighty-seven children with NRDS admitted to our hospital from June 2017 to June 2020 were selected as study subjects,and were divided into observation group(n=45)and control group(n=42)according to the random number table method.Children in the observation group were treated with new box-type oxygen,and children in the control group were treated with hood oxygen.2h after the completion of oxygen therapy,the serum indicators were reviewed,and the clinical treatment effect,serum CRP,TGF-β1,MMP-9,heart rate,respiration,oxygenation index,blood gas analysis indicators pH,PaCO2,partial pressure of carbon dioxide(PaCO2),partial pressure of arterial oxygen(PaO2)and oxygen saturation(SaO2),and the incidence of total adverse reactions were compared between the two groups.Results:After treatment,the total clinical effective rate of NRDS in the observation group was significantly higher than that in the control group(88.89%VS 71.43%,P<0.05).The serum levels of CRP,TGF-β1 and MMP-9 in the observation group were significantly lower than those in the control group(P<0.01).The heart rate and respiratory rate of NRDS children in the observation group were significantly lower than those in the control group,while the oxygenation index was significantly higher than that in the control group(P<0.01).There was no significant difference in PaCO2 between the two groups(P>0.05),and pH,PaO2 and SaO2 in the observation group were significantly higher than those in the control group(P<0.05).Conclusion:The new type of box oxygen supply is a safe and effective oxygen supply method,which can effectively reduce the inflammation state of NRDS children,improve the heart rate,respiration,oxygenation index and blood gas index,and improve the clinical efficacy.
