摘要
目的探讨彩色多普勒超声诊断仪行大脑中动脉及脐动脉监测在胎儿窘迫早期诊断中的价值及预后。方法将2016年10月至2019年9月收治的210例孕妇纳入本研究,将诊断为早期胎儿窘迫的95名孕妇作为观察组,将同期正常的115例孕妇作为对照组,两组孕妇皆行彩色多普勒超声检查,比较两组孕妇其胎儿大脑中动脉以及脐动脉的血流动力学指标,统计新生儿Apgar评分,观察新生儿不良结局。结果对照组脐动脉的PI、RI、S/D指数显著低于观察组(P<0.05)。对照组大脑中动脉的PI、RI、S/D指数明显高于观察组(P<0.05)。对照组的新生儿Apgar评分高于观察组(P<0.05)。对照组新生儿不良结局发生率为3.48%,显著低于观察组20.00%(P<0.05)。结论超声检测大脑中动脉以及脐动脉的血流指标可更为精准地诊断胎儿窘迫情况,值得在临床治疗中推广。
Objective To investigate the value and prognosis of middle cerebral artery and umbilical artery monitoring with color Doppler ultrasonography in early diagnosis of fetal distress.Methods From October 2016 to September 2019,210 cases of pregnant women included in this study.Ninety-five pregnant women diagnosed with early fetal distress were set as observation group,115 normal pregnant women as control group.All pregnant women were performed color doppler ultrasound examination.The blood flow dynamics index of middle cerebral artery and umbilical artery between two groups were compared.The neonatal Apgar score and neonatal outcomes were analyzed.Results The PI,RI,S/D indexes of umbilical artery in the control group were significantly lower than those in the observation group(P<0.05).The PI,RI and S/D indexes of the middle cerebral artery in the control group were significantly higher than those in the observation group(P<0.05).Apgar score was higher in the control group than in the observation group(P<0.05).The incidence of neonatal adverse outcomes in the control group was 3.48%,significantly lower than that in the observation group(20.00%,P<0.05).Conclusion Ultrasonic examination of blood flow index of middle cerebral artery and umbilical artery can diagnose fetal distress more accurately and is worth promoting in clinical treatment.
