摘要
目的观察采用首尾穴同取为主要手段治疗腰椎间盘突出伴坐骨神经痛的临床疗效。方法将符合腰椎间盘突出伴坐骨神经痛诊断标准的患者120例,随机分为治疗组(常规针刺加首尾穴组)、对照组(常规针刺组),每组60例。对照组取肾俞、大肠俞、委中、阳陵泉等穴,治疗组在与对照组取穴相同基础上针刺足太阳膀胱经或足少阳胆经首尾穴,观察2组的治疗即刻和治疗25 min后的临床镇痛效果。结果在治疗即刻和治疗25 min后,针刺首尾穴组VAS评分、JOA评分与对照组比较差异有显著统计学意义(P<0.01);直腿抬高试验首次治疗总有效率:治疗组为93.33%(56/60),对照组为25.00%(15/60),治疗组明显高于对照组。结论以针刺首尾穴为主要手段治疗腰椎间盘突出伴坐骨神经痛具有较好快速镇痛疗效,值得临床推广应用。
Objective To observe the clinical efficacy of the treatment of lumbar disc herniation with sciatica using acupuncture at the first and the last acupoints together.Methods 120 patients who met the diagnostic criteria for lumbar disc herniation with sciatica were randomly divided into the treatment group(conventional acupuncture combined with the first and the last acupoints group)and the control group(conventional acupuncture group).In the control group,BL23,BL25,BL40 and GB34 points were taken.The experimental group was acupunctured with bladder meridian of foot-taiyang or gallbladder meridian of foot-shaoyang through the first and the last acupoints on the same basis as the control group.The two groups were observed the clinical analgesic effect immediately and at 25 minutes after treatment.Results The VAS score and JOA score in the treatment group were significantly different from those in the control group.In the straight leg raising test after the first treatment,the total effective rate of immediate analgesia in the treatment group was 93.33%(56/60),which was significantly higher than25%(15/60)in the control group.Conclusion Acupuncture at the first and the last acupoints has shown good efficacy in treating lumbar disc herniation with sciatica.It is worthy of large-scale application.
