摘要
目的比较单孔腹腔镜及传统腹腔镜下治疗妇科良性病变的临床效果。方法选取2017年4月至2019年12月扬州市妇幼保健院妇科收治的168例卵巢囊肿、异位妊娠及子宫肌瘤等良性病变患者,均为女性,年龄(36.72±6.31)岁,年龄范围为18~55岁。根据手术方法不同将患者分为传统腹腔镜组(n=88)和单孔腹腔镜组(n=80),比较两组患者的手术时间、术中出血量、住院时间、并发症发生率及术后24 h疼痛评分。结果单孔腹腔镜患者手术时间[(72.97±15.61)min]长于传统腹腔镜组[(60.67±18.79)min],差异有统计学意义(P<0.01);单孔腹腔镜组患者住院时间[(4.00±0.60)d]少于传统腹腔镜组[(5.00±1.11)d],差异有统计学意义(P<0.01);单孔腹腔镜组患者术后24 h疼痛视觉模拟评分[(2.84±0.70)分]低于传统腹腔镜组[(3.90±0.70)分],差异有统计学意义(P<0.01)。结论常规器械行单孔腹腔镜安全可行,可明显减少妇科良性病变患者的术后疼痛,促进患者术后康复,提升术后满意度。
Objective To compare the clinical effects of laparoendoscopic single-site(LESS)surgery and conventional laparoscopic surgery for gynecological benign lesions.Methods A retrospective study was performed on 168 patients with gynecological benign lesions(benign tumor,ectopic pregnancy,uterine fibroids)who were admitted to the department of Gynecology at Yangzhou Maternity and Child Health Hospital from April 2017 to December 2019.The patients were all females,aged(36.72±6.31)years old,ranging from 18 to 55 years old.According to different surgical methods,patients were divided into the conventional laparoscopic surgery group(n=88)and the LESS surgery group(n=80),and the operative time,intraoperative blood loss,length of hospital stay,incidence of complications,and pain score 24 hours after surgery were compared between the two groups.Results The operation time in the LESS surgery group[(72.97±15.61)minutes]was longer than that in the conventional laparoscopic surgery group[(60.67±18.79)minutes],and the difference was statistically significant(P<0.01).The hospitalization time of the LESS surgery group[(4.00±0.60)days]was less than that of the conventional laparoscopic surgery group[(5.00±1.11)days],and the difference was statistically significant(P<0.01).The visual analogue score of pain 24 hours after surgery in the LESS surgery group[(2.84±0.70)points]was lower than that in the conventional laparoscopic surgery group[(3.90±0.70)points],and the difference was statistically significant(P<0.01).Conclusion Laparoendoscopic single-site surgery with conventional instruments is safe and feasible,which can significantly reduce postoperative pain in patients with gynecological benign lesions,promote postoperative rehabilitation of patients,and improve postoperative satisfaction.
