老年糖尿病患者肺炎及流感疫苗接种情况及成本效益分析 被引量:3

摘要 目的分析老年糖尿病患者肺炎及流感疫苗接种情况及成本效益,旨在为老年糖尿病患者肺炎及流感预防提供一种经济有效的手段。方法以回顾性队列研究法,抽取2017年10月~2018年4月接受罗湖区3所社区健康服务中心慢性病管理的老年糖尿病患者312例进行研究。将其按照是否联合接种肺炎及流感疫苗分为疫苗组和非疫苗组,各156例。比较两组呼吸道感染和抗生素使用情况,感染次数和诊疗费用,并对成本-效益实施分析。结果疫苗组呼吸道感染率及抗生素使用率分别为15.4%、7.1%,均低于非疫苗组的28.2%、19.2%(P<0.05)。疫苗组呼吸道感染次数少于非疫苗组(P<0.05)。疫苗组呼吸道感染抗生素使用费用秩均值以及诊疗总费用秩均值分别为148.86元、144.95元,均低于非疫苗组的164.16元、168.05元(P<0.05)。肺炎及流感疫苗接种效应为145339.49元,成本效应为3.23,净效益为100317.89元,人均净效益为643.06元。结论老年糖尿病患者肺炎及流感疫苗接种后可显著降低呼吸道感染以及抗生素使用率,成本效益较高,值得临床推广应用。 Objective To study and analyze the cost-effectiveness of pneumonia and influenza vaccination status in elderly patients with diabetes,in order to provide an economical and effective means for the prevention of pneumonia and influenza in elderly patients with diabetes.Methods A retrospective cohort study was conducted on 312 elderly diabetic patients who received chronic disease management in 3 community health service centers in Luohu District from October 2017 to April 2018.The patients were divided into 156 cases in the vaccine group and 156 cases in the non-vaccine group.Compared in terms of respiratory tract infection,antibiotic use,frequency of respiratory tract infection,and related diagnosis and treatment costs in two groups.And a cost-benefit analysis was conducted.Results The respiratory infection rate and antibiotic usage rate in the vaccine group were 15.4%and 7.1%,respectively,which were lower than 28.2%and 19.2%in the non-vaccine group(P<0.05).The degree of respiratory infection in the vaccine group was lower than that in the non-vaccine group(P<0.05).The rank mean of antibiotic use cost and total diagnosis and treatment cost in the vaccine group were 148.86 and 144.95,respectively,which were lower than 164.16 and 168.05 in the non-vaccine group(P<0.05).The effect of pneumonia and influenza vaccination was 145,339.49,the cost effect was 3.23,the net benefit was 100,317.89,and the net benefit per capita was 643.06.Conclusion Vaccination against pneumonia and influenza in elderly diabetic patients can significantly reduce the rate of respiratory infection and antibiotic use,which is cost-effective and worthy of clinical application.
