不同组织类型的小儿神经母细胞瘤自噬活性与免疫细胞水平的一致性及其临床意义 被引量:2

Consistency of autophagy activity and immune cell level in different types of neuroblastoma in children and its clinical significance

摘要 目的:观察不同组织类型的小儿神经母细胞瘤的自噬活性与免疫细胞水平的一致性并探讨其临床意义。方法:收集我院2008年8月~2018年8月期间收治的86例小儿神经母细胞瘤Ⅳ期患者,根据组织分型分成组织结构良好型(FH组,n=44)和组织结构不良型(UH组,n=42),收集患者的一般临床资料,对比两组患者治疗前后血清CD3+、CD3+CD8+、CD3+CD4+、NK细胞及B淋巴细胞水平;Western blot检测神经母细胞瘤内自噬分子LC3、Beclin-1和p62蛋白表达水平;Kappa分析观察血清免疫细胞水平与神经母细胞瘤内自噬分子LC3、Beclin-1和p62蛋白水平的一致性。结果:患儿的身体状态评价结果显示:FH组KPS<70分6例,KPS=70分11例,KPS>70分27例;UH组KPS<70分8例,KPS=70分14例,KPS>70分20例;两组患儿身体状态评分情况未见显著差异(χ^2=1.643,P=0.440)。两组样本中CD3+、CD3+CD8+、CD3+CD4+、NK细胞及B淋巴细胞水平显示UH组显著低于FH组(P<0.05)。Western blot结果显示,UH组LC3、Beclin-1和p62蛋白表达水平显著低于FH组,差异具有统计学意义(P<0.05)。一致性分析结果显示,FH组中自噬分子表达水平与免疫细胞水平具有显著一致性(Kappa=0.431,P=0.002);UH中自噬分子表达水平与免疫细胞水平不具有显著一致性(Kappa=-0.081,P=0.598)。结论:与UH组相比,FH组自噬活性与免疫细胞水平具有一致性,其免疫水平与肿瘤细胞自噬水平更具有相关性。 Objective:To observe the autophagy activity of pediatric neuroblastoma with different tissue types and its consistency with the level of immune cell,exploring its clinical significance.Methods:86 phaseⅣpatients with pediatric neuroblastoma admitted to our hospital from August 2008 to August 2018 were divided into favorable histology group(FH group,n=44)and unfavorable histology group(UH group,n=42)according to tissue classification.Two groups were collected for general clinical data.The serum levels of CD3+,CD3+/CD8+,CD3+/CD4+,NK cells and B lymphocytes were compared before and after treatment.Western blot technology was used to detect the expression levels of autophagy molecules LC3,Beclin-1 and p62 in neuroblastoma.Kappa analysis revealed that whether the levels of serum immune cells were consistent with the levels of autophagy molecules LC3,Beclin-1 and p62 in neuroblastoma.Results:The physical condition evaluation results of the children showed:6 cases with KPS<70 points,11 cases with KPS=70 points,27 cases with KPS>70 points in FH group,and 8 cases with KPS<70 points,14 cases with KPS=70 points.20 cases with KPS>70 in UH group.There was no significant difference in the physical status scores between the two groups(χ^2=1.643,P=0.440).The levels of CD3+,CD3+/CD8+,CD3+/CD4+,NK cells and B lymphocytes in UH group was significantly lower than those in FH group(P<0.05).Western blot analysis showed that the expression levels of LC3,Beclin-1 and p62 in UH group were significantly lower than those in FH group(P<0.05).The consistency analysis results showed that the expression level of autophagy molecules in FH group was significantly consistent with the level of immune cells(Kappa=0.431,P=0.002);while the expression levels of autophagy molecules and immune cell in UH group were not significantly consistent(Kappa=-0.081,P=0.598).Conclusion:Compared with UH group,the autophagy activity of FH group was more consistent with the level of immune cells,and its immune level was more correlated with the level of autophagy of tumor cells.
