摘要 目的探讨热射病患者早期(入院1 h内采集的标本)凝血功能与预后的关系及预测价值。方法回顾性分析某综合ICU 2014年5月至2018年10月就诊的62例热射病患者的临床资料,按照患者预后结局分为存活组(n=45)和死亡组(n=17)。收集热射病患者入住ICU后1 h内采集的血清样本的凝血功能指标,比较两组间的差异。采用单因素Logistic回归分析进行影响因素的初筛(变量筛选的检验标准设为a=0.05)。将筛选出的变量纳入二元Logistic回归分析及受试者工作特征曲线(ROC)进行分析,判断其对预后的预测价值。结果PT、APTT、TT、PTA均是影响热射病患者不良预后的危险因素(P<0.05)。根据ROC曲线分析PT对热射病病死率评估有一定价值。二元Logistic回归分析结果显示PT对热射病不良预后评估具有重要价值。结论早期监测PT可对热射病患者不良结局提供一定评估价值。 Objective To explore the relationship between coagulation function and prognosis in patients with heat stroke in the early stage(samples collected within 1 hour of admission)and its predictive value.Methods The clinical data of 62 patients with heat stroke treated in a comprehensive ICU from May 2014 to October 2018 were retrospectively analyzed.According to the prognosis,the patients were divided into survival group(n=45)and death group(n=17).The coagulation function indexes of serum samples collected within 1 h after hospitalization in ICU were collected,and the difference was compared between the two groups.Univariate logistic regression analysis was used to screen the influencing factors(the test standard for variable selection was set as a=0.05).The selected variables were included in multivariate logistic regression analysis and ROC analysis.Judging its predictive value for prognosis.Results PT,APTT,TT,PTA were risk factors for adverse prognosis of heat stroke patients(P<0.05).Analysis of PT based on ROC curve was valuable for evaluating the fatality rate of heat stroke patients.Multivariate logistic regression analysis showed that PT had important value in evaluating the adverse prognosis of heat stroke.Conclusion Early monitoring of PT can provide some evaluation value for adverse outcomes of heat stroke patients.
