摘要
目的评估家庭医生团队通过签约的模式对社区2型糖尿病患者进行健康管理的效果。方法选取2018年1月1日-2018年7月1日于西安市某医院就诊的2型糖尿病患者100例作为研究对象,采用随机数字表法将研究对象分为研究组和对照组,每组50例。家庭医生团队通过签约的模式对研究组患者进行健康管理,对照组患者进行常规门诊管理,对比2组患者的糖尿病认知情况、自我管理能力、体质指数及血糖控制情况。结果研究组患者健康管理后,其糖尿病认知、自我管理能力得分分别为(7.25±1.04)分、(6.56±0.84)分,均较健康管理前有明显提高(P<0.05),而体质指数、血糖指标(空腹血糖、餐后2 h血糖、糖化血红蛋白)分别为(23.88±2.74)kg/m2、(5.77±0.79)mmol/L、(9.26±1.36)mmol/L及(5.87±0.80)%,均较健康管理前有明显降低(P<0.05)。同时,研究组患者经健康管理后的糖尿病认知和自我管理能力得分还均较同时期对照组更高,而体质指数和血糖指标均较同时期对照组更低,且差异均具有统计学意义(P<0.05)。结论以家庭医生团队为主体,采用签约的模式对社区2型糖尿病患者进行健康管理可以明显提高糖尿病患者的认知和自我管理能力,改善患者的体质指数和血糖控制效果。
Objective To assess the impact of the family physician team on the health management of patients with type 2 diabetes mellitus in the community through contract signing.Methods One hundred patients with type 2 diabetes mellitus who visited a hospital in Xi'an from January 1,2018 to July 1,2018 were selected as the study subjects,and the subjects were divided into a study group and a control group with 50 cases in each group using a random number table.The patients in the study group were managed by a signed family physician team,while the control group was managed in a routine outpatient clinic.The cognitive status regarding diabetes mellitus,self-management ability,body mass index and blood glucose control of the two groups were compared.Results After health management,the cognition of diabetes mellitus and self-management ability scores of the patients in the study group were(7.25±1.04)and(6.56±0.84)points,respectively,which were significantly higher compared with those before health management(P<0.05),while the body mass index and blood glucose index(fasting blood glucose,2 h postprandial blood glucose,glycated hemoglobin)were(23.88±2.74)kg/m2,(5.77±0.79)mmol/L,(9.26±1.36)mmol/L and(5.87±0.80)%,respectively,all significantly lower than those before health management(P<0.05).Besides,after health management,the scores in the cognition of diabetes mellitus and self-management in the study group were higher than those in the control group,while the body mass index and blood glucose index were lower than those in the control group in the same period,and the differences were statistically significant(P<0.05).Conclusion The community health management of patients with type 2 diabetes mellitus with signed family physician team as the main body can significantly improve the cognitive and self-management abilities,body mass index and glycemic control of patients with diabetes mellitus.
