摘要
目的了解浙江省舟山市异性性途径感染1型艾滋病病毒(HIV-1)基因亚型和传播特征。方法收集舟山市2016-2018年新确证经异性性行为感染且未经抗病毒治疗HIV-1病例的样本,运用套式反转录荧光定量聚合酶链反应(RT-PCR)和巢式PCR方法扩增pol基因并测序,构建系统进化树分析基因亚型及成簇特征。结果47例样本成功获得38例合格序列,优势亚型为CRF08BC 16例,CRF01AE 11例,其他包括CRF07BC 7例、B亚型3例、CRF6701B 1例。发现4个样本存在1个监测性耐药突变,其中3例均为K103N且在同一传播簇,1例为M46L。发现包含22例病例的4个传播簇,传播簇中的病例≥50岁嫖客15例,其中最大传播簇由13例病例组成,均居住在岱山县衢山街道,含有10例嫖客和1例暗娼。结论舟山市异性性传播优势亚型为CRF08BC和CRF01AE,老年嫖客和暗娼是舟山市异性性传播网络中的重点人群,岱山县衢山街道是HIV-1感染病例分布密集的区域,应加强监测并重点对传播簇病例开展干预。
Objective To understand the subtypes and transmission characteristics of HIV-1 in heterosexual men and women in Zhoushan of Zhejiang province.Methods Blood samples were collected from the newly-diagnosed HIV-1 people infected via heterosexual transmission with treatment-naive in Zhoushan during 2016-2018.Pol gene was amplified and sequenced,the gene subtypes and transmission characteristics were analyzed by phylogenetic tree construction.Results A total of 38 sequences were obtained successfully from 47 samples.The predominant subtypes were CRF08BC(16 cases)and CRF01AE(11 cases),followed by CRF07BC(7 cases),B(3 cases)and CRF6701 B(1 case).Four samples were found to have drug resistant mutations(K103 N,M46 L)with 3 of K103 N in one transmission cluster and 1 of M46 L.There were 4 transmission clusters containing 22 cases,among which 15 cases were clients≥50 years old who hare Sex with female sex workers.The largest cluster was composed of 13 cases who lived in Qushan street of Daishan county,with 10 cases of clients and 1 case of female sex worker.Conclusion The predominant subtypes of HIV-1 are CRF08BC and CRF01AE.The clients≥50 years old and female sex workers are the key groups in HIV transmission,and the cases in Qushan street of Daishan county is more.Efforts should be made to strengthen surveillance and intervention for HIV/AIDS transmission.
