摘要
目的探讨层级全责一体化护理模式在糖尿病患者中的应用效果及对血糖水平的影响。方法选择2019年2月至2019年11月辽宁省精神卫生中心收治的100例糖尿病患者作为研究对象,随机分为对照组和观察组,每组50例。对照组患者在治疗期间采用常规护理,观察组患者在治疗期间采用层级全责一体化护理模式。比较两组患者护理干预后的餐后2 h血糖(2hPPG)、糖化血红蛋白(HbA1c)以及空腹血糖(FPG)水平,并评估两组患者的病情监测以及锻炼指导评分。结果护理干预后,观察组2hPPG、HbA1c及FPG水平均低于对照组,差异均有统计学意义(P<0.05);观察组患者的病情监测以及锻炼指导评分均高于对照组,差异均有统计学意义(P<0.05)。结论对糖尿病患者实施层级全责一体化护理可显著提升血糖控制效果,且对预防并发症的发生有积极的作用。
Objective To investigate the application eff ect of the integrated full responsibility nursing model in diabetic patients and its infl uence on blood sugar level.Methods A total of 100 diabetic patients admitted to the Liaoning Mental Health Center from February 2019 to November 2019 were selected as the research objects and randomly divided into a control group and an observation group,wit h 50 cases in each group.Patients in the control group used routine care during the treatment period,and patients in the observation group used a hierarchical full responsibility integrated care model during the treatment period.The 2 h postprandial blood glucose(2 hPPG),glycosylated hemoglobin(HbA1c)and fasting blood glucose(FPG)levels of the two groups after nursing intervention were compared,and the disease monitoring and exercise guidance scores of the two groups were evaluated.Results After nursing intervention,the 2 hPPG,HbA1c and FPG levels of the observation group were lower than those of the control group,and the diff erences were statistically signifi cant(P<0.05);the observation group patients'condition monitoring and exercise guidance scores were higher than those of the control group,and the diff erences were statistically signifi cant(P<0.05).Conclusion Implementing hierarchical full responsibility integrated care for diabetic patients can signifi cantly improve the eff ect of blood sugar control,and it has a positive eff ect on preventing complications.
