摘要
目的:比较腹腔镜前列腺剜除术(LP)与经尿道钬激光前列腺剜除术(HoLEP)治疗大体积前列腺增生症的安全性及临床疗效。方法:回顾性分析我院2016年3月~2019年12月40例前列腺增生症患者的临床资料,其中20例行LP,20例行HoLEP。比较两组患者术中血红蛋白变化、血钠水平变化、手术时间、住院天数、留置尿管时间、切除腺体重量、国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、残余尿(PVR)。结果:两组IPSS、Qmax、PVR、手术时间、住院时间的差异均无统计学意义(均P>0.05)。与HoLEP组相比,LP组血红蛋白下降幅度小、血钠影响小、切除腺体组织多、留置尿管时间稍长(均P<0.05)。结论:腹腔镜剜除术治疗大体积前列腺增生症安全有效,尤其适用于合并尿道狭窄和膀胱憩室需同时处理的患者。
Objective:To compare the safety and clinical efficacy of laparoscopic prostatectomy(LP)and transurethral holmium laser enucleation of prostate(HoLEP)in treatment of massive benign prostatic hyperplasia.Methods:The clinical data of 40 patients with benign prostatic hyperplasia in our hospital from March 2016 to December 2019 were retrospectively analyzed.Among them,20 cases underwent LP,and 20 cases underwent HoLEP.The following indexes were compared between two group,including changes in intraoperative hemoglobin and Na+level,operative time,hospital stay,indwelling catheter time,weight of removed glands,International Prostate Symptom Score(IPSS),maximum urine flow rate(Qmax)and post-void residual urine(PVR).Results:There were no significant deference in IPSS,Qmax,PVR,operative time and hospital stay between two groups(all P>0.05).Compared with the HoLEP group,the gland tissues were resected more,and the indwelling catheter time was slightly longer,with less decrease of hemoglobin and little effect on blood sodium in the LP group(all P<0.05).Conclusion:LP is safe and effective for treatment of massive benign prostatic hyperplasia,which is especially suitable for patients combined with urethral stricture and bladder diverticulum.
