摘要
目的探讨祛脂愈肝颗粒联合易善复对痰湿内阻型非酒精性脂肪性肝病(Non-Alcoholic Fatty Liver Disease,NAFLD)患者氧化应激指标及炎性因子的影响。方法选取2018年1月—2020年1月期间河南省安阳市中医院收治的痰湿内阻型NAFLD患者共100例,采用随机数字表法分为对照组50例和治疗组50例。对照组给予易善复治疗,在此基础上治疗组给予祛脂愈肝颗粒治疗,两组均治疗3个月。观察两组患者中医证候疗效,治疗前及治疗3个月后肝功能指标、脂代谢指标、氧化应激指标及炎性因子水平并进行统计学分析。结果治疗后治疗组总有效率90.00%(45/50),对照组总有效率76.00%(38/50),两组比较差异有统计学意义(P<0.05)。两组患者治疗后血清AST、ALT指标水平均低于治疗前(P<0.05),且治疗组治疗后血清AST、ALT均显著低于对照组(P<0.05)。两组患者治疗后血清TG、TC及LDL-C水平均显著低于治疗前,HDL-C水平显著高于治疗前(P<0.05),且治疗组治疗后血清TG、TC及LDL-C水平均显著低于对照组,HDL-C水平显著高于对照组(P<0.05)。两组患者治疗后血清SOD水平显著高于治疗前,血清MDA水平显著低于治疗前(P<0.05),且治疗组治疗后血清SOD水平显著高于对照组,血清MDA水平显著低于对照组(P<0.05)。两组患者治疗后血清TNF-α、IL-6水平均显著低于治疗前(P<0.05),且治疗组治疗后血清TNF-α、IL-6水平均显著低于对照组(P<0.05)。结论祛脂愈肝颗粒联合易善复能够改善痰湿内阻型NAFLD患者的中医证候,保护患者肝细胞,改善患者肝功能,减轻机体炎性反应及氧化应激反应。
Objective To investigate the effects of Qizhi Yugan Granule combined with Yishanfu on oxidative stress and inflammatory factors in patients with nonalcoholic fatty liver disease(NAFLD)with phlegm dampness.Methods A total of 100 patients with phlegm-dampness internal resistance type NAFLD who were admitted to Anyang Hospital of Traditional Chinese Medicine in Henan Province from January 2018 to January 2020 were selected and divided into 50 cases in the control group and 50 cases in the treatment group by random number table.The control group was given Yishanfu treatment,and on this basis,the treatment group was given Quzhiyugan Granules.Both groups were treated for 3 months.The efficacy of TCM syndromes in the two groups of patients was observed,and the levels of liver function indexes,lipid metabolism indexes,oxidative stress indexes and inflammatory factors before treatment and 3 months after treatment were statistically analyzed.Results After treatment,the total effective rate of the treatment group was 90.00%(45/50),and the total effective rate of the control group was 76.00%(38/50).The difference between the two groups was statistically significant(P<0.05).After treatment,the serum AST and ALT levels of the two groups of patients were lower than before treatment(P<0.05),and the serum AST and ALT levels of the treatment group after treatment were significantly lower than those of the control group(P<0.05).Serum TG,TC and LDL-C levels of the two groups of patients after treatment were significantly lower than before treatment,and HDL-C levels were significantly higher than before treatment(P<0.05),and serum TG,TC and LDL-C levels of treatment group after treatment Both were significantly lower than the control group,and the level of HDL-C was significantly higher than the control group(P<0.05).After treatment,the serum SOD level of the two groups of patients was significantly higher than before treatment,and the serum MDA level was significantly lower than before treatment(P<0.05),and the serum SOD level after treatment in the treatment group was significantly higher than the control group,and the serum MDA level was significantly lower than the control group(P<0.05).Serum TNF-αand IL-6 levels after treatment in the two groups were significantly lower than before treatment(P<0.05),and serum TNF-αand IL-6 levels in the treatment group after treatment were significantly lower than those in the control group(P<0.05).Conclusions Quzhi Yugan Granules can improve the TCM syndromes of NAFLD patients with phlegm-dampness internal resistance,protect the liver cells of the patients,improve the liver function of the patients,and reduce the body’s inflammatory response and oxidative stress.
