医院获得性感染人苍白杆菌的临床分布、危险因素与耐药性分析 被引量:4

Analysis of the clinical distribution,risk factors and drug resistance of hospital acquired infection caused by Ochrobactrum anthropi

摘要 目的探讨医院获得性感染人苍白杆菌的临床分布、危险因素与耐药性,为有效控制该菌感染提供参考。方法回顾性分析2011年1月-2019年9月金华市中心医院经细菌培养确诊为人苍白杆菌感染患者的临床资料。结果 94例人苍白杆菌感染患者主要为年龄≥60岁和≤5岁者,分别占35.1%和36.2%;患者主要来自农村(76.6%)。标本类型主要为血液(59.5%)。主要来自儿科、重症监护室,分别占30.9%、24.5%。引起该菌感染最主要的危险因素是手术、机械通气、静脉留置导管。药敏试验表明该杆菌对β-内酰胺类抗菌药物具有较强的耐药性,其耐药率均高于90.0%;对美罗培南、亚胺培南等抗菌药物的耐药率均低于10.0%;对头孢吡肟的耐药率为25.5%。结论人苍白杆菌主要引起来自农村的免疫力低下者发生血流感染,感染的主要危险因素是侵入性诊疗,治疗该菌感染首选美罗培南、亚胺培南和头孢吡肟。 Objective To investigate the clinical distribution,risk factors and drug resistance of hospital acquired infection caused by Ochrobactrum anthropi,and to provide reference for the effective control of the bacterial infection. Methods A retrospective analysis was made of the clinical data of the inpatients with bacterial infection caused by Ochrobactrum anthropi in Jinhua Central Hospital from January 2011 to September 2019. Results Ninety four cases of Ochrobactrum anthropi infection were mainly aged over 60 and under 5 years old,accounting for 35. 1% and 36. 2% respectively;the patients mainly came from rural areas,accounting for 76. 6%. Blood was the main type of specimens,accounting for 59. 5%. The patients mainly came from pediatrics and intensive care unit,accounting for 30. 9% and 24. 5% respectively. The main risk factors for the infection were operation,mechanical ventilation and indwelling catheter. Drug susceptibility test showed that the bacteria had strong resistance to beta-lactam antimicrobial drugs,and the drug resistance rate was higher than 90. 0%;the resistance rates to meropenem and imipenem were all lower than 10. 0%,and to cefepime was 25. 5%. Conclusion Ochrobactrum anthropi mainly causes blood stream infection in immunocompromised people from rural areas,invasive diagnosis and treatment is the main risk factors for the bacterial infection,meropenem,imipenem and cefepime are the first choice for the treatment of the bacterial infection.
