下颌阻生第三磨牙及其邻近结构三维有限元模型的建立及分析

摘要 目的通过建立下颌阻生第三磨牙及其邻近结构的三维有限元模型,对阻生牙拔除时受力进行模拟分析,为临床操作提供理论指导。方法利用CBCT获取DICOM数据,采用Mimics 16.0软件初步建立三维模型,采用Geomagic Studio 2014软件进行修补、降噪及曲面化。在Pro/E 5.0软件中将颌骨皮质骨和松质骨对应的几何实体模型组装。利用Hypermesh 13.0软件进行网格划分。最后在MSC.Patran/Nastran 2012软件中进行有限元网格二次处理及有限元分析。结果成功建立下颌阻生第三磨牙及其邻近结构的三维有限元模型,模型共计19494个节点、90720个网格单元。阻生牙的近中、远中、颊侧、舌侧、牙根处都有应力集中现象,其中远中及舌侧应力较大,且应力多数集中在牙冠外形高点以上。结论三维有限元法能够准确地模拟下颌阻生第三磨牙及其邻近组织的形态结构,并且能够对其进行模拟手术应力分析,对临床操作具有指导作用。 Objective To provide theoretical guidance for clinical operation by establishing a three-dimensional finite element model of the impacted mandibular third molar and its adjacent structures,and simulation and analysis of the force of the impacted teeth during extraction.Methods CBCT was used to obtain DICOM data,and Mimics 16.0 software was used to establish a preliminary three-dimensional model.Geomagic Studio 2014 software was used for repair,noise reduction and surface rendering.The corresponding geometric solid model of cortical bone and cancellous bone were assembled in Pro/E 5.0 software.The Hypermesh 13.0 software was used for grid division.Finally,the second processing and finite element analysis of finite element mesh were carried out by MSC.Patran/Nastran 2012 software.Results The three-dimensional finite element model of the impacted mandibular third molar and its adjacent structures were successfully established.The model included 19494 nodes and 90720 elements.The concentration of stress was found in the mesial,distal,buccal,lingual,and root of impacted teeth,of which the distal and lingual stress were greater,and most of the stress was concentrated above the height of crown contour.Conclusion The three-dimensional finite element method can accurately simulate the shape and structure of the impacted mandibular third molar and its surrounding tissues.It can be used to analyze the stress during tooth extraction and guide the clinical operation.
