摘要
目的探讨宫颈环扎术联合缩宫抑制疗法对宫颈机能不全双胎妊娠患者妊娠结局的影响。方法回顾性分析2017年1月~2019年10月大连市妇幼保健院收治的360例宫颈机能不全患者中78例双胎妊娠者的临床资料。将采用宫颈环扎术联合缩宫抑制疗法的48例患者归为观察组,将采用缩宫抑制治疗的30例患者归为对照组。记录观察组患者手术完成情况,比较观察组手术前后宫颈管长度,比较两组妊娠结局及并发症发生情况。结果观察组48例患者均顺利完成手术,手术时间为(35.23±5.64)min,术中出血量为(19.46±4.21)mL;观察组术后48例患者经阴道B超检查宫颈管长度为(1.98±0.32)cm,低于术前的(3.13±0.26)cm,差异有统计学意义(P<0.05);两组孕龄、新生儿体重比较,差异无统计学意义(P>0.05);观察组新生儿Apgar评分高于对照组,差异有统计学意义(P<0.05)。两组患者均未出现胎膜早破,宫缩无法抑制导致流产发生,无术后感染发生。结论宫颈机能不全双胎妊娠患者采用宫颈环扎术的妊娠结局较好,对胎儿发育不会造成影响。
Objective To investigate the effect of cervical cerclage combined with uterine contraction suppression on pregnancy outcome of cervical incompetence patients with twin pregnancy.Methods The clinical data of 78 cases of twin pregnancy in 360 patients with cervical incompetence who were admitted in Dalian Maternal and Child Health Hospital from January 2017 to October 2019 were retrospectively analyzed.A total of 48 patients who were treated with cervical cerclage and uterine contraction suppression during pregnancy were classified as the observation group,and 30 patients who were treated with uterine contraction suppression were classified as the control group.The conditions of patients in the observation group after surgery was recorded.The cervical canal length in the observation group was compared before and after surgery,and the pregnancy outcome and complications were compared between the two groups.Results A total of 48 patients in the observation group underwent surgery successfully,the operation time was(35.23±5.64)min,and the intraoperative bleeding volume was(19.46±4.21)mL.The cervical canal length of 48 patients in the observation group examined by vaginal bultrasound after surgery([1.98±0.32]cm)was lower than that before surgery([3.13±0.26]cm),with statistically significant difference(P<0.05);the gestational weeks and neonatal weight were compared between the two groups,and the differences were not statistically significant(P>0.05);the Apgar score of the observation group was higher than that of the control group,with statistically significant difference(P<0.05);premature rupture of membranes was not present in the two groups of patients,and the miscarriage was caused by uterine contraction cannot be suppressed,and no postoperative infection occurred.Conclusion Cervical cerclage for cervical incompetence patients with twin pregnancy has better pregnancy outcome,and it will not cause effect on fetal development.
