摘要
目的:探讨降钙素原(PCT)联合CD64指数对小儿肺部感染的诊断价值。方法:选取肺部感染患儿100例,通过血培养、痰培养等检查结果确诊的细菌感染患儿52例为Bacteria组,病毒感染患儿27例为Viruses组,支原体感染患儿21例为Mycoplasma组;另外选取呼吸系统健康儿童40例作为Blank组。对比分析各组血浆检测指标水平和阳性率等情况。结果:Bacteria组PCT、白细胞计数、C反应蛋白等指标水平及CD64指数均高于Mycoplasma组、Viruses组和Blank组,差异有统计学意义(P<0.05)。将Blank组和Bacteria组的PCT、CD64指数、WBC和CRP数据绘制ROC曲线,曲线下面积PCT(0.865)>CD64指数(0.842)>CRP(0.781)>WBC(0.769),与标准面积0.5比较,差异有统计学意义(P<0.05)。当PCT≥0.19μg/L时,灵敏度为90.38%,特异度为82.69%;CD64指数≥2.56时,灵敏度为88.46%,特异度为86.54%。结论:PCT联合检测CD64可以作为小儿肺部细菌感染的诊断的主要指标。
Objective:To explore the diagnostic value of PCT combined with CD64 index in children with pulmonary infection.Methods:100 children with pulmonary infection were selected,according to the results of blood culture and sputum culture,52 children with bacterial infection were diagnosed as the Bacteria group,27 children with virus infection were diagnosed as the Viruses group,21 children with mycoplasma infection were diagnosed as the Mycoplasma group.In addition,40 healthy children with respiratory system were selected as the Blank group.We compared and analyzed the levels of plasma test indexes and positive rate of each group.Results:In the baceria group,PCT,WBC,CRP and CD64 index were significantly higher than those in Mycoplasma group,viruses group and blank group,the difference was statistically significant(P<0.05).Plot receiver operating characteristic curves of PCT,CD64 index,WBC,CRP in Blank group and Bacteria group.The area under the curve PCT(0.865)>CD64 index(0.842)>CRP(0.781)>WBC(0.769),compared with the standard area 0.5,the difference is statistically significant(P<0.05).When PCT≥0.19μg/L,the sensitivity is 90.38%and the specificity is 82.69%;when the CD64 index is≥2.56,the sensitivity is 88.46%and the specificity is 86.54%.Conclusion:Combined detection of CD64 by PCT can be used as the main index for the diagnosis of pulmonary infection in children.
