超声心动图测量中晚孕正常胎儿瓣环位移差对评价胎儿心功能的临床价值研究 被引量:2

摘要 目的探讨超声心动图测量中晚孕正常胎儿瓣环位移差对评价胎儿心功能的临床价值。方法 227例中晚孕单胎孕妇,进行超声心动图测量,分析胎儿瓣环位移值、瓣环位移差、多普勒测量相关参数与各参数相关性;选取其中30例孕中期孕妇作为中孕组,另选取其中30例孕晚期孕妇作为晚孕组,对比两组组织运动瓣环位移自动追踪技术(TMAD)测量胎儿瓣环达峰时间和达峰位移。结果正常中晚孕胎儿M型超声心动图(FAM)-三尖瓣环位移(TAPSE)高于FAM-二尖瓣环位移(MAPSE),差异具有统计学意义(P<0.05);FAM-瓣环位移差(APSED)随孕周增加而增加,孕周、Sm、Em和Sm’、Em’与FAM-APSED呈正相关(r=0.717、0.886、0.897、0.918、0.833, P<0.05)。中孕组与晚孕组二尖瓣(MA)-达峰时间(TTP)、三尖瓣(TA)-TTP对比差异无统计学意义(P>0.05);中孕组TMAD-APSED、TMADMAPSE、TMAD-TAPSE分别为(1.14±0.20)、(2.86±0.59)、(4.00±0.67)mm,均低于晚孕组的(1.74±0.48)、(4.10±0.74)、(5.84±1.03)mm,差异有统计学意义(P<0.05)。结论超声心动图测量中晚孕正常胎儿瓣环位移差对评价胎儿心功能具有重要临床意义。 Objective To discuss the clinical value of echocardiographic measurement of normal fetal annular plane systolic excursion difference in middle and late pregnancy in evaluating fetal cardiac function. Methods 227 singleton pregnant women in middle and late pregnancy were measured by echocardiography to analyze correlation of fetal annular displacement, annular displacement difference, Doppler measurement and other parameters, 30 cases of them were randomly selected as the middle pregnancy group, and 30 cases of them were selected as the late pregnancy group. The peak time and peak displacement of fetal annulus measured by tissue motion annular displacement(tissue motion annular displacement) were compared between the two groups. Results For normal middle and late pregnancy fetus, the free angle M-mode(FAM)-tricuspid annular plane systolic excursion(TAPSE) value was high than FAM-(MAPSE) value;the value of FAM-annular plane systolic excursion difference(APSED) increased with the increase of gestational age, and the gestational age, Sm, Em and Sm’, Em ’were positively correlated with FAM-APSED(r=0.717, 0.886, 0.897, 0.918, 0.833, P<0.05). There was no statistically significant difference in mitral annulus(MA)-time to peak(TTP) and tricuspid annulus(TA)-TTP between middle pregnancy group and late pregnancy group(P>0.05). The TMAD-APSED, TMAD-MAPSE and TMAD-TAPSE of middle pregnancy group were(1.14±0.20),(2.86±0.59) and(4.00±0.67) mm, which were lower than(1.74±0.48),(4.10±0.74) and(5.84±1.03) mm of late pregnancy group, and the difference was statistically significant(P<0.05). Conclusion Echocardiographic measurement of normal fetal annular plane systolic excursion difference in middle and late pregnancy has important clinical significance in evaluating fetal cardiac function.
