摘要
目的探讨妊娠期糖尿病(GDM)孕妇超声心动图对胎儿心功能检查的临床意义。方法选取2019年1—8月福州台江医院收治的血糖控制不佳的GDM孕妇42例作为观察组,并选取同期在该院接受体检的健康孕妇60名作为对照组,应用超声心动图检查胎儿心功能,对其心脏各瓣环内径、心肌工作指数与心脏结构和功能参数等水平进行比较。结果两组左/右心室收缩末期内径(LVDs,RVDs)、左/右心室舒张末期内径(LVDd,RVDd)差异无统计学意义(P>0.05);但观察组的心脏三尖瓣环内径(TVA)、二尖瓣环内径(MVA)、肺动脉瓣环内径(PVA)、主动脉瓣环内径(AVA)均大于对照组;左室壁厚度(LVWT)、室间隔收缩末期/舒张末期厚度(IVSs,IVSd)均大于对照组;观察组的左室短轴缩短率(LVFs)、肺动脉/主动脉瓣血流峰值速度(PVPA,PVAO)、左心输出量(LCO)与左室射血分数(LVEF)均高于对照组;二尖瓣、三尖瓣E峰/A峰值速度(E/AMV、E/ATV)水平低于对照组;观察组左心室Tei指数显著高于对照组,以上比较差异有统计学意义(P<0.05)。结论血糖水平与胎儿心脏发育存在一定联系,对GDM孕妇通过开展超声心动图检查,可准确评价胎儿心脏功能情况,为及早诊断心功能受损提供临床依据。
Objective To explore the clinical significance of gestational diabetes mellitus(GDM)pregnant women with echocardiography in the examination of fetal heart function.Methods Select 42 pregnant women with poor blood glucose control in the hospital from January to August 2019 as the observation group,and select 60 healthy pregnant women who received physical examination in the hospital during the same period as the control group,and applied echocardiography to check the heart function of the fetus,and compare the inner diameter of the heart valve annulus,myocardial work index and the level of cardiac structure and function parameters.Results There was no statistically significant difference in the left/right ventricular end systolic diameter(LVDs,RVDs)and left/right ventricular end diastolic diameter(LVDd,RVDd)between the two groups(P>0.05);however,the tricuspid annulus inner diameter(TVA)of the observation group,the inner diameter of the mitral valve annulus(MVA),the inner diameter of the pulmonary valve annulus(PVA),and the inner diameter of the aortic valve annulus(AVA)were larger than those of the control group;left ventricular wall thickness(LVWT),end systolic/end diastolic thickness of the ventricular septum(IVSs,IVSd)were greater than those of the control group;the left ventricular short axis shortening rate(LVFs),pulmonary artery/aortic valve blood flow peak velocity(PVPA,PVAO),left cardiac output(LCO)and left ventricular ejection fraction(LVEF)were higher than the control group;the mitral and tricuspid valve E peak/A peak velocity(E/AMV,E/ATV)levels were lower than the control group;the left ventricular Tei index of the observation group was significantly higher than that of the control group.The differences were statistically significant(P<0.05).Conclusion There is a certain relationship between blood glucose level and fetal heart development.Echocardiography in pregnant women with GDM can accurately evaluate fetal heart function and provide clinical evidence for early diagnosis of impaired heart function.
