超声乳化联合房角分离术治疗急性闭角型青光眼合并白内障临床研究 被引量:6

Clinical Study of Phacoemulsification Combined with Angle Separation in the Treatment of Acute Angle-closure Glaucoma with Cataract

摘要 目的探究对急性闭角型青光眼合并白内障患者实施超声乳化联合房角分离术的治疗效果。方法将该院自2019年6月—2020年1月期间收治的58例急性闭角型青光眼合并白内障患者采取抽签法随机分为试验组(n=29)与参照组(n=29),参照组患者实施超声乳化联合小梁切除术,试验组患者实施超声乳化联合房角分离术,对比两组患者眼压、中央前房深度、最佳矫正视力及并发症发生情况。结果试验组患者眼压为(15.03±1.26)mmHg低于参照组的(16.19±1.39)mmHg(t=3.330,P=0.002),中央前房深度为(2.94±0.42)mm高于参照组的(2.18±0.36)mm(t=7.399,P<0.001),最佳矫正视力为(0.85±0.12)高于参照组的(0.64±0.09)(t=7.539,P<0.001),并发症发生率为6.90%明显低于参照组的31.03%,均差异有统计学意义(χ^2=5.497,P=0.019)。结论急性闭角型青光眼合并白内障患者实施超声乳化联合房角分离术能降低眼压,改善视力,增加中央前方深度,安全性高。 Objective To explore the effect of phacoemulsification combined with angle separation in patients with acute angle-closure glaucoma and cataract.Methods 58 patients with acute angle-closure glaucoma and cataract admitted to the hospital from June 2019 to January 2020 were randomly divided into the experimental group(n=29)and the reference group(n=29)by lottery.Patients in the reference group underwent phacoemulsification combined with trabeculectomy,and patients in the experimental group underwent phacoemulsification combined with angle separation.The intraocular pressure,anterior central depth,best corrected visual acuity and complications of the two groups were compared.Results The intraocular pressure of the test group was(15.03±1.26)mmHg,which was lower than the reference group(16.19±1.39)mmHg(t=3.330,P=0.002),and the central anterior depth was(2.94±0.42)mm,which was lower than the reference group(2.18±0.36)mm(t=7.399,P<0.001),the best corrected visual acuity was(0.85±0.12),which was higher than(0.64±0.09)in the reference group(t=7.539,P<0.001),and the complication rate was 6.90%significantly,which was higher than 31.03%in the reference group,the difference was statistically significant(χ^2=5.497,P=0.019).Conclusion Acute angle-closure glaucoma combined with cataract patients undergoing phacoemulsification combined with angle separation can reduce intraocular pressure,improve visual acuity,and increase central anterior depth.
